Rate: Queen $209.00 plus tax or Double/Double (2 beds) $249.00 plus tax per night. Make

Substitution/ Cancellation Policy
Make your reservation early! If your plans change and you cannot attend the activity, a colleague can attend in your place. Just call and let us know. Registration cancelled at any time
prior to January 1, 2015 is not subject to any fee. Cancellation after that time is subject to the

Accreditation:
This activity, New York Society of Addiction Medicine Annual Conference, with a beginning
date of January 30, 2015, has been submitted and if accepted, will be 14.50 prescribed credits by
the American Academy of Family Physicians. This activity has also be submitted to the ABAM for
credit for maintenance of certification.
AMA/AAFP Equivalency:
AAFP Prescribed credit is accepted by the American Medical Association as equivalent to AMA
PRA Category 1 Credit™ toward the AMA Physician’s Recognition Award. When applying for
the AMA PRA, Prescribed credit earned must be reported as Prescribed, not as Category 1.
