Welcome to Our Practice This booklet outlines the services that we offer. Please keep it safe and use it as a source of reference. NHS Durham Dales, Easington and Sedgefield Clinical Commissioning Group Who we are The Durham Dales, Easington and Sedgefield Clinical Commissioning Group (CCG) was established in October 2011 and authorised as a statutory body in February 2013. The CCG is made up of 40 member GP practices. It covers a total population of around 272,000 over a large and diverse geographical area. The CCG comprises three localities within County Durham: Durham Dales, Easington and East Durham, and Sedgefield. These localities have worked closely together as commissioners of local healthcare services for many years. Clinical commissioning means that local GPs across our localities will now have greater opportunity to use their knowledge about healthcare to develop services that meet our patients’ needs. Receptionists The receptionists have an important job and endeavour to deal with your queries, appointments, repeat prescriptions etc. as efficiently as possible. They have enquiries and calls coming from every direction, so please be patient. On occasions the receptionists may need to ask you a few details. They have been trained to make these enquiries in order that we can help you in the most appropriate way. Practice staff are bound by the same rules of confidentiality as Doctors. Practice Nurses We have a team of Practice Nurses and Health Care Assistants. Practice Nurses deal with blood tests • cervical smears • ear syringing • immunisations • blood pressure • family planning checks • removal of sutures • dressings as well as the following specialised clinics: Asthma COPD Diabetic Hypertension Heart Disease Smoking Cessation District Nurses The District Nurses work with the Practice in providing nursing care, advice and support to patients too ill to attend the surgery. *They can be contacted via the Practice Community Midwives They offer full maternity and post-natal care. An antenatal clinic is held at the Practice and patients will be visited at home as and when necessary during pregnancy and the post-natal period. Health Visitor The Health Visitor works in close liaison with the Practice concentrating on families with children under 5 years of age especially new babies. A baby clinic is held at the three sites at the following times: Wheatley Hill/Thornley alternate Tuesdays 2:00 – 4:00pm Kelloe the first Wednesday of every month 2:00 – 3:15pm Phoenix Medical Group 5 Updated 12-Nov-14 3. Patient Information Disabled Patients There is wheelchair access to all three sites at the front entrances. Patient services are all provided at ground floor level. Surgery Hours The Surgery is open for enquiries and to make appointments, etc., at the two main sites (Wheatley Hill Practice and Thornley Practice) Day Surgery Monday 8:00am – 6:00pm** Tuesday 8:00am – 6:00pm Wednesday 8:00am – 6:00pm Thursday 8:00am – 12:00 noon and 8:00am – 6:00pm Alternate Thursdays between Wheatley Hill & Thornley Friday 8:00am – 6:00pm **The Practice is open for extended hours (pre-booked appointments only) from 6:00pm – 8:00pm Mondays. From 23rd November 2013 for 17 weeks, the practice will be offering morning appointments as part of a winter pressures campaign. Please contact the surgery for more information. Doors are closed for lunch between 12:30pm and 1:00pm every day, but still available for enquiries, appointments, etc. Kelloe opens at the following times: Day Surgery Monday 8:30am – 12:30pm Tuesday 8:30am – 12:30pm Wednesday 8:30am – 12:30pm Thursday 8:30am – 12:00 noon Friday 8:30am – 12:00 noon Appointments Appointments can be booked more than two days ahead. We also have systemonline for anyone who has the internet, ask reception for a password, you can book GP appointments and also order repeat prescriptions (New for 2013). If you have already registered with the practice, click on the image. Standard consultation time is for 10mins but if you feel you need longer you may ask the receptionist for a longer appointment. These can be arranged by telephone or by calling in person. You can see the doctor of your choice but on occasions this will not always be possible. If you think your problem is medically urgent ask yourself, ‘Does my problem need to be seen today or do I want it to be seen today?’ Please give the receptionist an outline of your problem, including details of how long you have had the problem, so that she can organise an appropriate appointment or seek advice from the Doctors. Cancellations If you are unable to attend an appointment, which you have booked, please inform us as soon as possible so that your appointment can be given to someone else. If you fail to inform us that you no longer need your appointment or that you are unable to attend patient access is greatly reduced. Phoenix Medical Group 6 Updated 12-Nov-14 The Rights and Responsibilities of Patients Please keep all fire exits and disabled spaces clear at all times. Parents are asked to maintain control of their child to avoid accidents and disturbing other patients. The use of mobile phones in the surgery is prohibited. We expect patients to be considerate to our staff and fellow patients at all times. Sometimes GP appointments over run for reasons beyond our control, we ask for your patience and understanding. Patients who are rude, aggressive and violent or who persistently fail to attend appointments will be asked to seek care elsewhere. We have an open door policy for accepting new patients, providing they live within our catchment area and are eligible for treatment. If however, a patient is not accepted for another reason, a written explanation will be provided within one working week. Violence and Aggression We operate a permanent zero tolerance policy toward violent and abusive behavior. Any violent or abusive behavior – Physical or mental - could result in removal from the practice list. Your Medical Records You have a right to keep your personal health information confidential between you and your doctor. A patient leaflet is available from reception, which explains: How your medical records are used to help you. How the confidentiality of your records is safeguarded, whether stored in written or electronic (computer) format. That your records are readily accessible to authorized staff only. How you can arrange to see your own records Telephone Advice If you would like to speak to a Doctor / Nurse for advice but are unable to attend the surgery, please contact the receptionist who will be able to advise you of an appropriate time for a telephone consultation. If possible, give an indication of what you would like to discuss (unless there is something confidential which you would prefer to discuss with the Doctor). Home Visits If you are too ill to attend the surgery you can request a home visit by telephoning the Surgery, if possible before 10:30am. This allows the Doctors to plan their day. The receptionist will ask for details of your illness to help the Doctor assess the urgency of the visit. You will also be asked for a contact telephone number as the Doctor may wish to speak to you. Home visits are at the discretion of the Doctor and are for patients who are medically unable to attend the surgery. Remember, the Doctor can see four patients at the surgery in the time it takes to do one home visit. If your problem is urgent, you will always be seen the same day, so please try to come to the surgery if you can. (We have a home visit policy, which is available at the surgery). Out Of Hours Services Out of hours (CONTACT TEL NO. 111) service is provided by North East Ambulance service and organised through County Durham and Darlington Durham PCT. Please make proper use of the night/weekend service. Remember you can always see a Doctor in the next surgery so try to restrict night and weekend calls to genuine emergencies that cannot wait until the next day. The service is used from 6.00pm until 8.00am the following day. At weekends cover will be from 6.00pm Friday until 8.00am Monday they will also provide Bank Holiday cover. Urgent Care Centres are open 24/7 and are situated at: Bishop Auckland Hospital Darlington Primary Care Trust Shotley Bridge Community Hospital Peterlee Community Hospital Phoenix Medical Group 7 Updated 12-Nov-14 Open Monday to Friday 8:30am – 5:30pm: Seaham Medical Centre Open Monday to Friday 6:00pm – 8:00am and Friday from 6:00pm to 8:00am inclusive: University Hospital of North Durham If there is any delay in responding to an absolute emergency, you are instructed to dial 999 and ask for the Ambulance Service. Repeat Prescriptions If you are taking regular medication you may request a repeat prescription either by bringing your repeat prescription slip to the surgery, putting it in the box provided in Reception or posting it to us. Please allow 2 working days’ notice when requesting a repeat prescription. You will be requested to have your repeat medication reviewed regularly. If you are unable to pick up your prescription, you can send a stamped addressed envelope and your prescription will be posted to you. A delivery service is available but can only be arranged via your local Chemist. Prescriptions will not be given to children under 16 years. Please note: repeat prescriptions requested on a Friday will not be ready until Tuesday. WE DO NOT PRESCRIBE ANTIBIOTICS FOR COUGHS AND COLDS Test Results Wherever possible, please contact the surgery between 12 noon and 4.00pm for test results. The receptionist will either advise you of your results, once they have been cleared by the Doctor or ask you to speak to or make an appointment with the Doctor or Practice Nurse. Newly Registered Patients New patients are very welcome. The Practice does not discriminate against race • gender • social class • age • religion • sexual orientation, or appearance • disability or medical condition. When registering you will be asked to complete a GMS1 form and given an appointment with the Practice Nurse for a new patient medical health check, during which we will take details of your medical history and current medication, which will assist us while we are waiting for your medical records to arrive. Change of Address We are required by the Health Authority to notify them of any change of any name, address or telephone number, it is important you inform us as soon as possible so that our records can be updated. Services Provided Family Planning Care The Practice provides a comprehensive, confidential family planning service including: Prescribing oral contraception (the pill) Fitting Coils and Implants Giving the Contraceptive Injection Prescribing Emergency Contraception – please arrange to see a Doctor within 72 hours if you require this. Please make an appointment to discuss. Maternity Care Full ante-natal and post-natal care services are available. Please telephone for details. Child Health Services Baby Clinic: Alternative Tuesday afternoons at Thornley/Wheatley Hill Kelloe : Every other Wednesday from 11:30am – 12noon. The clinic, which is run by the Doctor, Practice Nurse and Health Visitor, is intended to provide complete health care for your child from birth to 5 years. This includes immunisations, developmental checks and general advice. It is important that your children are fully immunised against all childhood diseases. You will be sent a reminder when an injection is due. If you are unable to attend the appointment given, please contact us and we will organise another appointment. Minor Operations Run by Doctor and Nurse at the Thornley site. Warts and verrucae treated by freezing with liquid nitrogen Cervical Smears We recommend that smears are taken every three years (unless advised otherwise). The Practice Nurse will carry this out. Diabetic / Asthmatic / Other Chronic Diseases, e.g. Coronary Heart Disease We provide comprehensive care for those suffering from chronic diseases, e.g. diabetes, asthma/COPD, coronary heart disease, hypertension etc. Smoking Cessation If you would like to stop smoking please contact the surgery to arrange an appropriate appointment. Weight Management Please contact the surgery for details. We are now able to refer to Slimming World. Well Persons’ Examination If you would like a health check, please make an appointment with the Practice Nurse. Counselling We have a Practice-based counselling service that your Doctor can refer you to. Bereavement Counselling can be arranged via the surgery. Non-NHS Medical Examinations We are happy to carry out any non-NHS medical examinations, e.g. Heavy Goods Vehicle, Pre-Employment etc. The receptionist will be able to advise you of the fee. Travel Injections If you are travelling abroad you may require immunisations. Please make an appointment with the Practice Nurse approximately 8 weeks before you are due to travel. Complaints This practice offers a complaints procedure in accordance with NHS guidelines. Please speak to our Practice Manager or a Receptionist who will provide you with further information.
