Milwaukee School of Engineering 2014-2015 Verification Worksheet Your 2014-2015 Free Application for Federal Student Aid (FAFSA) was selected for a review process called “Verification.” In this process, MSOE will compare the information you reported on the FAFSA to the documentation requested. Please submit all required documentation within two weeks of receiving this worksheet, if possible. MSOE is aware that accessing the IRS Data Retrieval Tool (IRS DRT) can sometimes take more than two weeks; however, your financial aid will not be determined and disbursed until verification requirements are completed. MSOE is required to review the requested information under 34 CFR, Part 668. What to do: 1. Complete steps 1 – 7 and sign the Verification Worksheet. Make sure to carefully read all instructions as errors can delay the processing of your financial aid. 2. Submit the completed worksheet along with copies of 2013 IRS Tax Return Transcripts (if requested- See Section 3) and any other documents to the MSOE Financial Aid Office. 1. The FAFSA must be signed electronically with the student’s and parent’s PINs and re-submitted for processing. **To obtain a 2013 IRS Tax Return Transcript online: 1. Go to http://www.irs.gov/Individuals/Get-Transcript and click on the “Get Transcript ONLINE” link. 2. Create an account and follow the prompts. 3. You will be asked your reasoning for requesting a transcript. You can indicate “Higher Education/Student Aid.” 4. Please select “2013” under the Return Transcript section. Please submit the transcript(s) to our office. It can be faxed to 414-277-6952 or emailed as a PDF to [email protected] You can also order a transcript by phone at 1-800-908-9946. Make sure to request the “IRS Tax Return Transcript” and not the “IRS Tax Account Transcript.” Lastly, you can mail a request by completing an IRS 4506-T form.
