THE SWT SHP for intellectual excellence Director Ros Wolfers Building C, 8 Columbia Way Norwest Business Park Baulkham Hills 2153 Ph: (02) 9634-2000 Fax: (02) 9634-2842 Principal Anne Hodgkins ABN 31 050 562 902 APPLICATION FOR ENTRY TESTING – OC 2015 – FINAL TESTING DATE For 2015, all Year 4 students will need to sit the OC Entry Test prior to being offered a position in this programme. Those who are currently enrolled in a Swot Shop class should not use this application as you will be contacted directly by us with dates for the internal testing. The assessment will consist of two 30 minute papers similar to those used for the OC exam but the difficulty level will be appropriate for those who have just completed Year 3 at school. (Students will also be required to produce a piece of writing in case they want to be admitted to an English programme.) Our OC Placement programme will begin at the end of January and run for two terms (with an optional Review programme held in the July holidays just prior to the exam). Each lesson runs for two hours and all-inclusive fees for the 20 lessons are $1035.00 (weekday classes) or $1150.00 (weekend classes). The classes will focus on teaching and learning – not simply on questions and answers. Skills will gradually be developed in the areas to be tested: comprehension, maths and general ability. In addition students will be provided with activities which improve their speed, accuracy, concentration and exam technique. Short practice tests based on the previous lessons will be offered at least once each fortnight. Because each lesson builds on the one before, no students will be admitted to the programme after the first week. The classes are graded to ensure appropriate teaching techniques but students with poor scores on the entry test will not be admitted as the lessons would not be of benefit to them. I understand that: late students or students without a confirmation/receipt cannot be admitted to the examination parents will be provided with raw test scores for their child but decisions relating to admission to the programme rest totally with the Principal and Director the examination papers cannot be released or shown to parents applications must be received by us at least five days before the test date and will be processed subject to vacancies. The date and time for your child’s assessment is: Last opportunity to gain entry to OC Exam Preparation Programme starts Monday 26 January 2015 Thursday January 22nd 3:00 pm – 4:30 pm FOR CREDIT CARD PAYMENTS Please charge $25.00 to my credit card: _ _ _ _ / _ _ _ _ /_ _ _ _ Parent’s signature: __________________________ Expiry date: _ _ / _ _ Date: _____________________ Name: ___________________________ Child’s Name: _____________________________ Signature: ________________________ PLEASE ENSURE BOTH PAGES ARE RETURNED TO US.
