C O N S U LTAT I O N O P T I O N S Updated: 2/6/2015 CONSULT OPTIONS Phone Numbers are available by going to KaleidaScope and choosing Practitioner Search (http://icred/PhysSearch/) in the ‘Kaleida Links’ menu on the top left of the home page. Enter the first and last name of the physician, and then click ‘Search.’ A phone number will display. Weekend/Holiday Consult Requests: many consultants on weekends are covering several hospitals in addition to BGMC. Please telephone the consultant yourself for all urgent consult requests, to explain the reason for the urgency. PLEASE NOTE: Often, the on-call consultant is shown in the KaleidaScope “On-call Search” program. Navigate to this by clicking on the link http://kaleidascope/OnCallSearch/oncall_srch.asp or opening KaleidaScope and then clicking on “On-call Search” on the left in the listing of “Kaleida Links.” Choose ‘Buffalo General’ in the facility pane, and then select a specialty in the ‘Groups’ pane. In complex or time sensitive cases, in addition to entering an order, it is optimal to communicate directly with your consultant. 2. The consult order must specify the name of the consultant requested (exceptions are Allergy, Anesthesia, Neurology, P a i n M a n a g e m e n t , Psychiatry, University Cardiology, University Nephrology, University Pulmonary, and University Rheumatology, as on each of these services, there is one group of physicians who see the patient based on a predetermined schedule). 3. If the need for completion of the consultation is urgent, this must be specified in the order. For all urgent consults, direct physician to physician communication is best practice, in addition to the written order. 4. Since most Attendings prefer that consultants enter their recommendations as orders, please specify in the consult order if you wish “opinion only.” 5. Requesting Attendings typically prefer that consultants should NOT write an order for additional consultants without first discussing with the Attending. 6. Requesting Attendings also typically prefer that consultants should NOT write orders unrelated to the reason for the consult without first discussing with the Attending. 7. If consultant is in doubt as to whether the Attending would agree with orders (i.e. “controversial” treatment or diagnostic test, order outside of the field of expertise of the consultant), consultant should discuss with the Attending prior to entering an order. 8. Consultants seeing a patient on a teaching service should take the time to contact the resident directly to discuss recommendations for additional diagnostics or treatment. This is important with regard to house staff education. Each teaching service has a team ASCOM phone, a n d t h e ASCOM numbers are available in ‘On Call Search’ on KaleidaScope. 9. Consultants are requested to specify in consult notes which recommendations could be pursued on an outpatient basis post discharge versus which recommendations should be pursued prior to discharge. 10. Residents rotating on a consult service should not write orders until they have discussed the orders with the consult service Attending. CONSULT CATEGORIES 1. Allergy and Immunology – University/UB MD Call 859-2985 Mon – Fri, 8 a.m.-4 p.m. After hours, the Allergy Fellow on-call should be paged through the switchboard. Doctors Julian Ambrus Jr, Mark Sands (on call only), and Stan Schwartz do these consults as per a schedule. 2. Anesthesiology – Maplegate Consider consultation for prospective surgical patients, to evaluate options for proceeding to operating room. At this time, anesthesiology does not offer pain management consults. Call 859-2226 Mon - Fri 8 a.m. – 5 p.m.; after hours, page the Anesthesia Resident, at 643-0776. 3. Bariatrics Doctors Butsch (John, Jr), Hoffman (Aaron), and Posner (Alan) a. Consultation for advice or follow-up of an inpatient that has had bariatric surgery at BGMC - consults should be called to 859-3196; the consult will be assigned to the appropriate physician by the office. If the patient is currently a patient of the Kaleida program, please inform the office as to which surgeon performed the bariatric procedure. b. Inpatient bariatric surgery initial consultations are generally NOT available. The initial evaluation of a patient for bariatric surgery is a comprehensive, multidisciplinary process that is not suited to inpatients. In exceptional circumstances, if an Attending physician wishes an initial evaluation of an inpatient for bariatric surgery, the Attending must personally communicate this request to one of the bariatric surgeons. 4. Cardiology (see also ‘Electrophysiology’ and ‘Heart Failure’ categories below) a. BMG i. Doctors Bhayana (Ranjan), Carlson (David), Farhi (Eli), Ford-Mukkamala (Laura), and Morris (William) - consult most often completed by the physician you request. ii. Doctors Luisi (Andrew) and Spangenthal (Edward) – provide weekend coverage, with weekday consults generally completed by one of their colleagues. b. Buffalo Cardiology and Pulmonary - Doctors Banifatemi (Reza), Cobler (JoAnne), Corbelli (Richard), D’Angelo (Michael), Dashkoff (Neil), Gatewood, (Robert), Kozlowski (Lisa), Krawczyk (Justine), Matthews (George), Riegel (Brian), Rozmus (Grzegorz), Sobieraj (Scott), Wadhwani (Jay) and Zizzi Jr (Joseph). The consult will be completed by the doctor on the call schedule for consults. c. Buffalo Heart Group - Doctors Banas (Michael), Gelormini (Joseph), Gianfagna (Robert), Hong (Michael), Iacona (Marie), Jennings (Richard), Khan (Irfan), Madhusudanan (Mohan), Masud (Zaki), Meholick (Alan), Mehta (Vinay), Sullivan (Philip), and Wadhawan (Sachin). The consult will be completed by the doctor on the call schedule for consults. d. University/UB MD - Doctors Alfaro-Franco (Carina), Beck (Hiroko), Canty (John), Chaskes (Michael), Cimato (Thomas), Curtis (Anne), Fernandez (Stanley), Glover (Robert), Gokhale (Rohit), Graham (Susan), Iyer (Vijay), Malhotra (Saurabh), Page (Brian), Sharma (Umesh) and Zlotnick (David) - call 8593756 and leave a voice message; the consult will be completed by the doctor on the call schedule for consults. Evenings and weekends, if the consultation is urgent, page the on-call cardiology consult fellow (contact information posted on ‘On-Call Search’ on KaleidaScope). e. Dr. Conley (James) f. Cardiothoracic Surgery (the consult generally will be completed by the specific surgeon you request) a. Dr. Ashraf (Hasmat): focus on cardiac surgery, does not do major thoracic surgery b. Dr. Aldridge (Janerio): performs major cardiac and major thoracic surgery c. Dr. Demmy (Todd): focus on thoracic oncology d. Dr. Dexter (Elisabeth): focus on thoracic oncology e. Dr. Grosner (Gary): focus on cardiac surgery, does not do major thoracic surgery f. Dr Hennon (Mark): focus on thoracic oncology g. Dr Huang (Miriam): focus on thoracic oncology h. Dr. Levinsky (Leon): primarily device insertion and removal, does not do major thoracic or cardiac surgery i. Dr Nwogu (Chukwumere): focus on thoracic oncology j. Dr. Picone (Anthony): focus on thoracic surgery, does not do major cardiac surgery k. Dr. Takita (Hiroshi): focus on thoracic surgery, does not do major cardiac surgery l. Dr. Von Fricken (Kurt): focus on thoracic surgery, does not do major cardiac surgery 6. Colorectal Surgery a. BMG i. Doctors Attuwaybi (Bashir), Matier (Brian), Singh (Amarjit), and Visco (Jeff) - consult most often completed by the physician you request. ii. Doctors Butler (Bryan), Cywinski (Matthew), and Leberer (Joseph) – provide weekend coverage, with weekday consults generally completed by one of their colleagues b. Delaware Surgical – Doctors Adams (Timothy), Barone (Steve), and Falvo (Mark) c. University/UB MD – Dr. Kulaylat (Mahmoud) 7. Dental a. Contact the dentist of your choice. To determine which dentists have Attending privileges at Kaleida, go to KaleidaScope ‘Practitioner Search’ (http://icred/PhysSearch/) and enter ‘Dentistry’ from the drop down list in the ‘Specialty’ field. Those dentists whose ‘Status’ is listed as ‘Active’ are on staff and qualified to do inpatient consults. b. Administrative prior authorization is required for permission to transport an inpatient to the ECMC Dental Clinic for evaluation – contact Dr Colin McMahon, Medical Director of Hospital Operations, BGMC. 8. Dermatology Consultations are not routinely available. The most effective means of securing inpatient consultation is for the requesting Attending to discuss the patient condition directly with a dermatology Attending with whom the requesting physician has a professional relationship. In select situations in which an allergy might be the cause of the dermatologic condition, consider requesting an allergy consultation. 9. Dietary The patient will be seen by the dietitian assigned to the ward. NOTE: there are two RDs on duty on weekends, and a RD is available on-call on holidays. Dietary consultation may be useful in the following circumstances: a. Enteral nutrition recommendations/guidance b. TPN (HAL) recommendations/guidance c. Diet modification/supplementation guidance d. Counseling/patient and family education e. 10. Electrophysiology a. BMG - Doctors Kim (Chee), Switzer (Donald), and Erica Patino, NP – the consult will be completed by the physician with the most availability at BGMC that day. b. Buffalo Cardiology and Pulmonary - Dr. Rozmuz (Grzegorz) c. Buffalo Heart Group - Dr. Khan (Irfan) d. Electrophysiology @ General Physician - Dr. Bhatia (Ashish) and Smith (Nicole), NP e. University/UB MD – Dr. Beck (Hiroko) 11. Endocrinology a. BMG - Dr. Torre (Joseph) b. University/UB MD – Doctors Batra (Manav), Chaudhuri (Ajay), Dandona (Paresh), Kuhadiya (Nitesh), and Makdissi (Antoine). The consult will be performed by the physician on the call schedule. 12. ENT a. Urgent consults – doctors Callanan (Vincent) and O’Connell (Kevin). Call 608-1052 and the answering service will contact the physician on call. On Call Schedule is posted in On Call Search, http://kaleidascope/OnCallSearch/oncall_srch_results.asp b. Routine consults i. Doctors Callanan (Vincent) and O’Connell (Kevin) as per a call schedule. ii. Dr. Belles (William) iii. Dr. Muscarella (Joseph) 13. Ethics • Consults should be paged to 629-1487; if there is no response within 20 minutes, please call 522-5939. The on- call person will call back and obtain the patient’s name, room number, reason for consultation, and name of person requesting the consultation. • • • Ethics referrals are screened by the on-call person. Ethics consultations involve convening a small committee. These are available only Monday – Friday, and often 24 hours is required from the time of the request to organize the committee, as well as the family. For urgent matters, the Attending physician should ask the switchboard operator to page the risk management person on-call or the administrator on-call. 14. Family Medicine – University/UB MD Request a family medicine consult if the PCP is one of the family medicine inpatient service Attendings: Doctors Figueroa (Jeannette), Fox (Chet), Okazaki (Saburo), Patel (Vinod), Patnaik (Priyanka), Rosenthal (Thomas), Yale (Sandra), and Yang (Min) 15. Gastroenterology a. Buffalo Niagara GI - Doctors Corasanti (James) and Gupta (Alok); and Alicia Vasiloff, PA, Monica Wideman, FNP, and Alan Ziobro, FNP. Call consults to 859-4400. b. University/UB MD – Doctors Bou-Abdallah (Jad) and Mahl (Thomas). Perform consults only in weekend/vacation coverage for Dr. Corasanti c. Dr. Wells (Theodore) – performs consults only on existing patients d. Dr. Williams (Aston) – performs consults only on existing patients 16. Geriatrics Consultations are not routinely available. The most effective means of securing inpatient consultation is for the requesting Attending to discuss the patient condition directly with a geriatrics Attending with whom the requesting physician has a professional relationship. 17. Gynecology To determine the Attending and resident on call for a consult, with contact information, go to OnCall Search (http://kaleidascope/OnCallSearch/oncall_srch.asp) 18. Gynecologic Oncology a. Heart Failure • Weekdays – Judy Griffin, NP, and Terry Darlak, NP, in conjunction with Dr Susan Graham. Call consults to 859-1784. • Weekends (and Dr Graham vacations) – heart failure consultations will be performed by the University Cardiologist on the consult service. 20. Hematology (see also “Oncology” below) a. BMG - Doctors Cheng (Yijun), Conway (James), Yosuico (Victor), and Zielinski (Robert) - the consult will be completed by the doctor on the call schedule for consults. b. RPCI – Doctors Bhat (Seema), Czuczman (Myron MD), Griffiths (Elizabeth), Hernandez (Francisco), Liu (Hong), McCarthy (Philip), Thompson (James), Wang (Eunice), and Wetzler (Meir) c. University/UB MD – Dr. Romanowski (Roslyn) d. Dr. Islam (Abul) 21. Hospice (see also ‘Palliative Care’ below) All requests for Hospice are called to 686-8000. If the call drops to voice mail, please provide patient name, room number, name of the ordering physician or person making the request, and pertinent phone extension. 22. Hospitalist Consult – see ‘Family Medicine’ above and ‘Medicine’ below 23. Imaging Services - see “Radiology” below 24. Interventional Radiology (for additional information, see the section on ‘Radiology’ below and the section on ‘radiology procedures’ in the Practitioner Orientation to BGMC (https://www.infoclique.com/ic/resources/Practitioner-Orientation-toBGMC.pdf) document 25. Infectious Disease NOTE: it is requested for all weekend consults that the ordering physician speak directly with the on-call ID specialist. a. BMG - Dr. Brass (Chris) – vascular surgery and orthopedic surgery consults only. b. RPCI – Doctors Almyroudis (Nick) and Segal (Brahm) – WEKEND COVERAGE ONLY for UB MD. c. University/UB MD – Doctors Claus (Jonathan), Merchant (Shehzad) and Osawa (Ryosuke) – the consult will be completed by the doctor on the call schedule for consults. d. Dr. Cumbo (Thomas) 26. Medical Rehab Unit Consult will be completed by the physiatrist on the call schedule for that day. Consults should be called to the rehab office at 859-2562. Medicine – inquire as to the PCP, and consult the PCP Admission Relationship Lookup (http://kaleidascope/documents/PCP-‐Admissions/) to determine who should be consulted a. Occupational Therapy • Occupational Therapy should be ordered for patients going to 5N/MRU. • Occupational Therapy may be indicated in patients with orthopedic or neurological conditions. • Occupational Therapy should NOT be ordered automatically with physical therapy or ordered as part of an evaluation for SAR. 33. Oncology (see also ‘Gynecologic Oncology’ and ‘Hematology’ above) a. Krackow (Kenneth), McGrath (Brian), Phillips (Matthew), Rachala (Sridhar), and Santilli (Mario): write ‘University orthopedics consult,’ and the patient will be evaluated initially by house staff and then discussed with the Attending orthopedist on the schedule for consults. Janet Federick (859-1256) coordinates the schedule. After hours, page the on-call resident through the switchboard, or use the ‘On Call Search’ on KaleidaScope to determine whom to contact. ii. Spine: Dr Hamill (Christopher) e. Dr. Nenno (Donald): general orthopedics – cross covers with University/UB MD orthopedics. 37. Pain Management Gosy and Associates – Doctor Gosy (Eugene), Deborah Dzielski, NP and Kelly Jones, NP. Consults should be entered in PowerChart. 38. Palliative Care • Call 989 – 2390. Physiatry Consult Doctors Jerzewski (Aaron), Kuruvilla (Philip), and Schleifer-Schneggenburger (Jill) • • • These are distinct from a MRU consult, based on indication. Specify the indication for the consult with the order. Consult is entered in PowerChart (and may be called to the rehab office at 859-2562). 40. Physical Therapy • Physical therapists are here Monday – Friday 7:30 a.m. to 5:00 p.m. Orders for PT are entered in Cerner by nursing. To speak directly with a physical therapist after 4 p.m., call ASCOM 859-4963. • Physical therapists do work Saturday and Sunday 7:30 a.m. – 4 p.m. but routinely see only MRU, neurology and orthopedic patients, based on limited staffing. PT will see non-ortho inpatients on weekends only if called (8595164), and only if an evaluation is needed to clear the patient for discharge. • Be certain there is an appropriate activity order (i.e. not a bedrest order) when you order physical therapy for ambulation. • Physical therapy needs to be reordered in the following circumstances (as per government regulations): -Transfer to a higher level of care (i.e. transfer from med/surg bed to telemetry, ILCU or one of the ICUs). -After surgery requiring general anesthesia. 41. Wound Care Evaluations are done by clinical nurse specialists – enter in CPOE “Referral to wound/enterostomal therapist” and then specify the reason for the referral. • Stat evaluations are available 7 days per week – the nursing office has a schedule with the pager number of the nurse on-call each day for Stat evaluations. • Documentation by the wound care team can be viewed in the ‘Ancillary Documents’ clinical folder of the EMR. Wound dimensions and descriptions are also available in Results Review in the EMR.
