What happened?
The IP noticed the poly IBC for the klampress was nearly empty. He tried to lift one end to tilt the IBC to use the
last of the poly. Whilst doing this operation he felt a sharp pain in his lower back.
Immediate actions to make the area safe:
The IP stopped this activity and the area was made safe. The IP called the Out of Hours Coordinator who
informed the Duty Manager. IP went to a local NHS walk in Centre was prescribed pain killers and told to rest.
Safety message:
Never lift outside of your physical capabilities when undertaking any manual handling tasks and always ensure
that you follow your manual handling training techniques. Ensure to report all incidents to your Line Manager
immediately.

What happened?
The IP was shovelling rag off the ground into skip when he felt a sharp pain in his back.
Immediate actions to make the area safe:
The IP stopped this activity and waited for line manager to attend site. Line manager took IP to NHS walk in
centre. IP was prescribed pain killers and told to rest.
Safety message:
Never lift outside of your physical capabilities when undertaking any manual handling tasks and always ensure
that you follow your manual handling training techniques. Ensure you report all incidents to your Line Manager
immediately.

What happened?
A Marine Operative injured his right knee whilst stepping off a barge onto a pontoon bedded due to low tide.
The IP did not appear for work the following day due to a swollen knee and requires further assessment of the
extent of the injury.

Access to the area was restricted following the incident.
Safety message:
Ensure there is safe Access/Egress to places of work eliminating hazards

What happened?
The IP was carrying rubbish bags down a stair case, misjudged their footing and fell down two steps landing
on their front. IP believed to have twisted their ankle and was taken to the nearest A&E walk in centre for
treatment.
Immediate actions to make the area safe:
The stairs are in good condition with good lighting and clear of any obstacles. No action was required.
Safety message:
When ascending or descending stairs ensure you holding on the handrail do not carry heavy objects, do not
use your mobile or carry hot drinks

The IP was walking across the compound to the stores area when he lost his footing and went over on his
ankle, sustaining a sprain to his right ankle.

The area was checked to ensure that there were no other trip hazards.

Ensure any trip hazards are removed immediately adopting no compromise mentality. Ensure thorough site
inspections are carried out and actions arising are completed. When walking across a site always look where
you are walking and report any hazards.

What happened?
An operative hurt his back whilst manually handling scrap metal into a skip. The IP did not appear for work the

Always look at ways manual handling can be avoided but if needed ensure correct manual handling techniques
are used when lifting loads in accordance with the training given.

What happened?
The IP was using a hammer and cold chisel to prepare existing concrete for repair. The IP's hammer had a
rubber grip handle, the handle and metal part of the hammer came apart causing the hammer to hit the back
of his other hand causing bruising.
Immediate actions to make the area safe:
The hammer was withdrawn from service and discarded.
Safety message:
Prior to use ensure all tools are in good condition and are fit for purpose for the job at hand. Any tool found
faulty should be immediately withdrawn from service. Remember to take a Zero Comprise approach.

What happened?
The IP was walking up the stairs to the lime screw conveyor when he caught his foot on one of the steps. He
lost his balance and fell on the stairs hand railing, bruising his ribs. He continued working for the rest of the day
and the following day. He visited his GP on Thursday, who advised he needed to rest for a couple of days.
Immediate actions to make the area safe:
The stairs where in good condition and clear of any obstacles. No action was required.
Safety message:
When ascending or descending stairs ensure you holding on the handrail do not carry heavy objects, do not use
your mobile or carry hot drinks and do not run

What happened?
The operative was picking up a pump from the site and injured his hand on the asset tag ring when manoeuvring
the pump in position to be lifted by the Hiab. A laceration between the thumb and first finger occurred and it was
only when the operative removed his latex gloves that he noticed he had been cut.
At the time the wound was cleaned and dressed with the operative’s first aid kit, however over night the wound
became infected and when he visited the medical centre the wound was cleaned, dressed and antibiotics
administered.
Immediate actions to make the area safe
The asset tag ring was removed from the pump and replaced.
Safety message
Ensure that an assessment is undertaken prior to undertaking any manual handling activity.

What happened?
A technician was burnt on the side of the leg by hot sludge whilst unblocking a temporary pump. The pump
was being used to transfer sludge from the return activated sludge well to the Rotonox tank number 5.
Immediate actions to make the area safe:
The pump was immediately isolated and the IP applied a cold pack to the burn.
Safety message:
When isolating equipment to work on it, consider other sources of energy, not just electricity. In this case,
temperature and pressure.

What happened?
One of our colleagues arrived on site to collect his works van at 05:40; he got out of his own vehicle, noticed a
patch of ice and walked around it to his van. He started his van and got out to de- ice the windows, in doing so
he slipped on the ice previously avoided. As a consequence of this slip he fractured his left shoulder.
Immediate actions to make the area safe:
The car park was gritted using the fully stocked grit bin close to hand (less than 5 metres from where the
incident occurred)
Safety message:
Do not walk by, your safety is everyone’s priority, we must all assist in putting hazards right. When you are the
first person on site and notice an icy patch use the grit bins provided to make the area safe for yourself and
everyone else.

The IP was using his CAT & Genny to locate buried services when a driver ignored the "no entry" signs and
clipped IP whilst driving against the road closure.
Immediate actions to make the area safe
Works stopped and Police were called and the site was left safe.
Safety message
It is also essential all activities must be properly planned and subject to a risk assessment, undertaken by
trained and competent personnel. Work must be continually monitored for changing conditions that may
affect the ongoing safety of the activity. It is also essential works are carried out as per the risk assessment and
works plan; worksite is set-up correctly before activities commence team members are briefed before work
starts.

What happened?
During secondary lining concrete operations in the tunnel, an operative sustained a trapping / abrasive injury
to the tips and nails of the two middle fingers on his left hand. The IP was given first aid and the wound
cleaned up. IP went to A&E for further treatment and assessment.
Immediate actions to make the area safe:
The concrete re-mixer was checked for abnormalities/protrusion that could have caught IPs fingers. Nothing
out of the ordinary was found and the re-mixer was operating correctly.
Safety message:
Ensure access to nip points of machinery is prevented and warning signs are observed.

What Happened?
IP was attending a visible leak, reported by the Police to Thames Water, in the Guildford area GU8; during the
investigation the IP slipped on black ice and hurt his back.
Immediate actions to make the area safe:
IP informed PD of ice issue and requested site of leak to be gritted ASAP.
Safety message:
It is essential that an assessment of the work area is conducted; ensuring a SHE 5 / point of work risk
assessment is completed before work starts and ongoing as work develops and new hazards are realised. This
includes the weather condition, SHE 5 / point of work risk assessment, which should include the surrounding
environment, routes, paths and roads that are used for accessing the work site.

What Happened?
The IP, whilst attempting to lift a manhole cover, found it to be beyond his physical capabilities, the IP
experienced pain in his back and ceased the activity. Although he was able to continue with normal planned
work activities for the remainder of the day, the pain became progressively worse which resulted in the IP
having to seek medical advice and due to the discomfort; he was forced to rest his back leading to an absence
from the workplace.
Immediate actions to make the area safe:
No actions were required at the time of the incident as the activity could not be continued due to the cover
being too heavy for the IP to lift.
Safety message:
No matter how simple the activity might appear, everyone should consider the Task, Individual, Load and the
Environment (T.I.L.E.) before attempting to undertake any manual handling tasks.

The IP was stepping out of an excavation and in doing so stepped on some rubble that had been excavated
from the trench, the rubble moved under the IP’s weight causing him to fall and twist his ankle.

During your work activities always ensure that your working environment is free from any trips, slips and falls
that could have an impact on your health and safety. Remember to take a Zero compromise approach.

What happened?
Whilst the IP was attempting to connect a trailer to the hitch of his vehicle, the trailer rolled forward and
collided with him. This resulted in a fracture to his tibia and fibula.
Immediate actions to make the area safe:
The area was made safe and the trailer removed from site for safety checks; all CBJV trailers have been
quarantined until safety checks have been completed.
Safety message:
It is essential that vehicle checks are completed before leaving your home base, and if the job requires you to
tow a trailer, checks need to be made to confirm the trailer is fit for purpose. Checks must include the entire
unit (vehicle and trailer) after hitching and connection has been completed. It is essential that an assessment
of the work area is conducted; ensuring that when hitching a trailer you are on flat ground and the parking
systems is engaged. It is also essential that on-going assessments are carried out on reversing vehicles, heavy
trailers, work area hazards; and the need to check (vehicles and trailers) before driving.

The IP tripped on an electrical mat that had been moved from its normal position in front of electrical cabinets
during previous works. It was placed in a main walkway, and whilst not particularly proud, was enough for the
individual to trip and sustain ligament damage to his foot.

The matting was replaced to its correct position.
Safety message:
Housekeeping is everyone’s responsibility – if we move things temporarily to undertake works, we should
ensure that we replace items correctly. If we see something that we know is out of place, do not walk on by, do
something about it.

a pick-up truck. A cover that was laying on top of another cover into the frame. He misjudged the timing to
remove his hand before lid fell into place trapping his finger.

The site was left safe and the IP went to A&E.

A Modebest Builders Ltd carpenter went to step from one side of the working deck to the other, forgetting he
was still clipped on via his lanyard. As the IP took the step, the lanyard jerked him backwards causing him to
lose balance and fall into a void striking his rib area against formwork.
Immediate actions to make the area safe:
The site was made safe to ensure no one else was at risk of injury following the reporting of the incident.
Safety message:
Unsafe conditions and acts lead to incidents. Ensure there is safe access and eliminate voids where ever
possible.

What happened?
A contractor undertaking refurbishment works on site was walking back to his vehicle. Hoses for tankering and
jetting were laid out over the bank, next to the road and had potentially dislodged some stones on to the road.
The injured person tripped on a large stone on the road twisting his knee, resulting in 4 days lost time.
Immediate actions to make the area safe:
All stones were removed from the road surface.
Safety message:
Many slips and trips result from the smallest of hazards and this incident highlights the imperative for ensuring
the work area is clean and tidy, and devoid of debris that could cause someone to slip or trip.

What happened?
Scaffolders were dismantling a Haki stair structure at the bottom of the north east side of the Pumping shaft;
when a short piece of toe board fell from a height of 1.5m and struck another scaffolder on his hard hat. As a
precautionary measure the IP was taken to hospital for a check-up. He was released after examination and did
not return for his normal shift.
Immediate actions to make the area safe
No one else was at risk of injury following the incident.

What happened?
At site 4 two operators were moving the top section of a lifting bridle located on timber bites on the floor
surface. During the process the device became unbalanced and toppled over onto its side and on to the foot of
one of the men. He was wearing the correct safety boots but the shackle hit the injured person above the steel
toe cap injuring his foot.
Immediate actions to make the area safe:
The site was made safe to ensure no one else was at risk of injury following the incident.
Safety message:
Lifting equipment should not be left in an unsafe position and must be checked prior to commencing the task.

What happened?
Two operatives, toward the end of their shift, attended site for the purpose of replacing a defective manhole
cover and frame. The two operatives were manually carrying the cover and frame toward a flatbed vehicle.
Whilst performing this task the cover section fell out from the frame and landed on the feet of both operatives,
with the resulting injury being a fractured bone to the foot of one of the IP’s.

The area was made safe prior to returning the equipment to the depot and then transporting the injured
operatives to the local hospital.
Safety message:
Consider the task, individual, load and environment when undertaking manual handling tasks and be wary of

What happened?
The IP was attending the pumping station to carry out a planned cleaning operation of the wet well. Prior to
undertaking the works, the IP needed to access the control panel to isolate the pumps.
It was whilst making his way to the control panel that the IP placed his weight on a step which gave way and
caused the IP to sustain a knee injury.
Immediate actions to make the area safe:
The area was made safe and the injured operative visited the local hospital.
Safety message:
Prior to commencing any work activities of a similar nature, identify hazards which could potentially give rise to
an injury and be particularly mindful of changes in level within the work area which may pose a slip or trip
hazard.

What happened?
A Thames Water Operator was moving waste from the powerhouse to the waste yard, using a Manitu. The
skip was overloaded and his vision was slightly obscured, resulting in a blind spot. The driver noticed that an
item had fallen from the skip in his mirror; at this stage he had not identified that a third party was
approaching in a vehicle. He stopped and as he drove off he veered onto the opposite side of the road colliding
with a car driving in the other direction. This resulted in the front wing of the car being substantially damaged
and the driver sustaining whip lash.
Immediate actions to make the area safe:
The area was cornered off, and full investigation was carried out.
This process of waste removal has been stopped and a stand down set for 21/10/14.
Safety message:
Ensure that the appropriate plant and equipment is used for the task.
When using plant ensure you have full visibility, and where necessary the use of a banksman.

What happened?
The IP entered the welfare facilities for the purpose of removing his work clothes and whilst removing his hard
hat, part of the cuff on the sleeve of his Hi-visibility coat rubbed against his eye. No immediate effects were felt
and as a result the IP felt no need to seek first aid treatment and felt capable of driving home.
Overnight the IP was in some discomfort from the eye infection which forced him to seek medical advice at his
local A & E. The advice given was to rest the eye for 48hrs and to apply a prescribed antibiotic ointment.

The area was reviewed on the day and the hole was back filled with spoil immediately to form a level landing

Engineer was taken to hospital where he received immediate treatment for the injury but was retained
overnight on a drip of antibiotics to protect against any infection / contamination.
Safety message
The individual received a relatively minor injury as a result of the gloves he was wearing – had no or incorrect
PPE been used, the injury could have been much more substantial. Ask yourself – am I properly protected!!
Similarly, confirm that you are using the correct tools or equipment for the role – in this case the company had
recently identified jubilee clips with a wing nut wheel that could be turned by hand, rather than using a slotted
head and screwdriver.

What happened?
An operative was attending a blockage activity. In order to access the manhole he had to access the customer's
property which had a disabled access ramp to the front door. After discussing access with the customer he
retreated down the ramp and as he stepped off the side of the ramp he went over on his left ankle. He was
taken to hospital by his field manager where an x-ray confirmed no broken bones but a badly swollen ankle.

What happened?
The injured person was working at Abbey Mills Chamber A2 as a Carpenter. He was working in the chamber
near the carpenter bench and stepped on a piece of timber which was not properly de-nailed, resulting in the
nail puncturing the sole of his safety footwear. The IP was sent to hospital for a Tetanus injection.
Immediate actions to make the area safe
Work was stopped and the area was tidied and made safe.

In the Line of Fire - Hands should be kept a safe distance away from cutting equipment in motion and blades
should be adequately guarded to prevent contact with the blade, and ensure that the items being worked on
are secured into position.

What happened?
The network team were setting up to clean storm tanks at Auckland Road. The IP was stepping up onto the
walkway to enter the tanks when they caught his foot on the end of the walkway and fell forward onto the
metal walkway grating. His knee hit the grating splitting it open. This required treatment at hospital and a
number of stiches.

All changes in height on site have been checked and highlighted with spray paint as necessary.

What happened?
An operative working for a delivery contractor was is the process of securing the tarpaulin cover upon an
articulated trailer, when he fell from a height of approximately 3 metres onto a concrete surface.
IP was discovered by a second operative working in the same area, and an Ambulance was called to site. IP was
treated with Morphine, at the scene, and was taken to hospital.

Area was made safe, to prevent any other vehicle entering the area.

A leakage technician was working within the boundary of a customer’s property undertaking a customer side
leak investigation. The technician slipped on some uneven block paving, turning his ankle. He didn’t consider it
to be serious until he took his work boot off and it started to swell. He visited the hospital where he was
diagnosed with a sprained ankle and told to rest.

The site conditions were believed to be safe to work within.

It’s important that you check your surroundings before commencing work, especially where the work
environment is an unfamiliar one. This should be considered as part of the point-of-work risk assessment (TW
SHE5), and extra care taken if the area is uneven.

What happened?
An operative was attending a blockage clearance job, and had finished jetting and was removing the hose from
the chamber when it became stuck. In order to remove the hose he stepped back from the cover he was
standing on for stability and his foot became stuck in mud surrounding the chamber. As the operative
attempted to free his foot, he lost his balance. He fell injuring his knee.
Immediate actions to make the area safe
The operative’s assistant made area safe.
Safety message
This incident highlights the importance of risk assessing your environment. The muddy wet conditions in which
this type of activity is undertaken is not uncommon and this could cause slips or trips.
These conditions need to be taken into account during the point of work risk assessment, and for appropriate
actions taken to mitigate against them in the safe system of work.

During night shift two fitters were removing a slurry pipe bracket within the slurry treatment plant de-sander

What happened?
The IP was moving screenings with a manual pallet truck inside the screening house when he slipped on the
wet tiled floor. He hit the middle of his back on the outlet penstock, causing bruising and discomfort. The floor
had been cleaned and excess water removed just prior to the task commencing.
Immediate actions to make the area safe
The floor was already clear of excess water. This task would normally use a powered electric pallet truck
but at the time it was faulty, this has now been repaired.
Future use of the manual pallet truck has been stopped, and the purchase of a standby electric pallet truck
initiated.

When equipment fails, alternative methods / equipment should be fully assessed to identify previously hidden
hazards.

What happened?
The IP was unlocking a gate when he stepped on a plastic manhole cover. The cover dislodged and his left leg
went into the hole causing injuries to his lower leg.

Work was stopped. The manhole frame and cover was replaced and Heras fence erected around the manhole
to ensure that the location was safe. A survey was immediately carried out to ensure that there were no other
similar instances. Two identical locations were quickly found and marked. The project team will permanently
secure all 3 manhole covers with a concrete slab.
Safety message
Ensure all pedestrian walkways and workplaces are safe, conduct a survey not only the workplace but also the
pedestrian routes to the workplace and eliminate any potential slip and fall hazards.

A specialist contractor was carrying out the replacement and upgrade of a flow Inverter, to an isolated existing
MCC panel. Whilst undertaking this task the IP removed a protective cover which housed live electrical
components and in doing so came into contact with live 415v resulting in an electrical flash which burned the
IP’s face, neck and left hand.
Immediate actions to make the area safe
The area was made safe; the IP was taken to hospital and is in a stable condition. It is believed he will make a
full recovery.
Safety message
Ensure all electrical works have the necessary authorisations (TWOSA/ TOCOP), permits and isolations in place.
Ensure that there are trained competent persons carrying out the works and that all persons have been briefed
and fully understand the safe systems of work and necessary controls.

What happened?
A Network Engineer was undertaking daily van checks on his vehicle. Whilst he was topping up the screenwash reservoir, the bonnet of the vehicle dropped, trapping his hand. He attended A&E to have the injury
checked, and whilst the x-rays indicate no broken bones, his hand has swollen up, severe enough for him to be
admitted to hospital pending potential surgery.
Immediate actions to make the area safe
The vehicle, a Transit Connect, has a fixed stay to hold the bonnet open. This is to be inspected to check for
mechanical faults. All team members were texted by the line manager to ensure they were aware of the
incident, and to check their own vehicles, ensuring they were extra vigilant when undertaking their own
checks.
Safety message
When working under the bonnet of a vehicle, ensure that the mechanical stay is firmly engaged into the
locating lugs; avoid placing your hands in the “line of fire” where they may be struck should the stay fail. Be
aware of the weather conditions you are working in, and where it is windy, especially with hydraulic stays, try
and locate the vehicle out of the direct wind.
