PAYMENT METHODS
It is our goal to make payments easy and flexible. Orders
may be billed to your Visa, MasterCard, American Express or
Discover Card when placing your order. If you prefer you may
have orders billed to your account that may be paid by check or
monthly auto billing on your credit card. All unpaid accounts
receivable past due are subject to a finance charge.
DELIVERY TERMS
Freight charges will be applied based on a customer’s BRAVO
program reward level. Orders will be shipped via standard
UPS ground transportation unless otherwise requested.
Additional carrier charges related to special delivery services
and hazardous material shipments may also be applied.

If you have tried a product and it is defective or does not
perform to your satisfaction, we will gladly do what it takes
to make it right. Items returned for credit after 60 days are
subject to a twenty percent (20%) restocking fee. No credit
will be issued on products returned after six months from date
of purchase.

The Prescription Drug Marketing Act (PDMA) requires any
customer returning prescription drugs to complete and return
a Prescription Drug Return Authorization form. Federal law
requires that the healthcare entity returning prescription
drugs document that the product was kept under proper
storage and handling conditions while in their possession and
during return of the product. To get a copy of the form, please
contact Burkhart Customer Service at 800.562.8176.

You can search and access your Safety Data Sheets
(SDS) on the Burkhart online store. Simply go to
store.burkhartdental.com. For best results, log into the
store to search for SDS’s specific to your order history. If you
do not have log-in credentials for the Burkhart online store,
