PLAYERS & OFFICIALS RELEASE AND WAIVER OF LIABILITY
NOTICE: This form contains a release and waiver of liability and when signed is a contract with legal consequences.

TO: The United States of America Cricket Association hereinafter referred to as USACA and Atlantic Region Cricket Board
hereinafter referred to as ARCB :
In consideration of the opportunity afforded to me to participate in the USACA and Atlantic Region Cricket Board Activities
named below, I freely agree to, and make the following contractual representations and agreements:
I, the undersigned participant, acknowledge that I am participating in this activity at my own risk and that USACA and ARCB is in
no way responsible for me. I do hereby knowingly, freely and voluntarily assume any and all liability for any damage or injury
which may occur as a result of my participation in the activity and agree to release, hold harmless, indemnify, waive, discharge,
and covenant not to sue USACA and ARCB, its Regional Administrations, officers, agents, employees, sponsors and volunteers
from any and all liability or claims which may be sustained by me or to any person or any property, directly or indirectly, in
connection with or arising out of, my participation in the activity, whether caused in whole or in part by negligence of USACA and
ARCB or otherwise.
I, the undersigned participant have read this form, fully and understand its terms, and understand
that I, on behalf of myself have given up substantial rights by signing it and have signed it freely and
without any inducement of assurance of any nature and intend it to be a complete and
unconditional release of any and all liability to the greatest extent allowed by law and agree that if
any portion of this contract is held to be invalid, the balance notwithstanding shall continue in full
legal force and effect.
ARCB and USACA Activities;
Trials for USACA 2015 U13 National Tournament to be held in NJ, PA and VA in May 2015 through National Tournament
to be held Chicago, IL from July 9-12, 2015, including any associated travel, training and practice activities for the
duration period.
I have read the above waiver and release and by signing it agrees it is my intention to exempt and relieve
USACA and ARCB from liability for personal injury, property damage or wrongful death caused by negligence or any other cause
