What you should know
• T o protect your privacy, winners will be
selected by Optum two weeks after the
end of each quarter. If you’re a winner,
you will be contacted by Optum, and
you will need to consent to provide Shell
with your name to claim your prize.
• T o have your name entered into the
Contest, your wellness check must be
completed and received* by Optum
between October 1, 2014, and
September 30, 2015. Entrants will not
be included until Optum has received
the completed wellness check*. Each
quarter, names will be selected in
a random drawing of all entries.
Non-winning names will remain in
future drawings until the Contest ends.
