1. Pursuant to the above references, please advise following-named personnel that they
are being directed to undergo the Fire Basic Recruit Counse (FBRC) at the National Fire

2. Please advise them to process their Medical and IAS ClearanceS 0n ,lt ilay 2015
and they can have their laboratory exams (ECG, Chest X-ray, Urinalysis, CBC, Serum
Pregnancy Test - for women) at any government-owned or accredited hospitals or clinics.
Validation of laboratory resutts as well as medical evaluation shall be done by the Regional
Medical Officer at the Regional Headquarters.
3. Hence, they are expected to report at this office for the issuance of clearances. For
monitoring purposes, they are advised to submit photocopy of the following to Training
Section, CDD on 25:.29 IUay 20tS prior to reporting to the NFTI, to wit:
a. Appointment
b. MedicalCertificate
c.
RegionalNHQ IAS Clearance
4. Further, listed hercunder are the documents that the concerned BFP personnel shall
submit to the Chief Registrar, NFTI upon reporting thereat:
a.

Duly accomplished Personnel Data Sheet (PDS) wslhZvZ colored lD with
headgear
b. Medical and Dental Certificate
c. IAS Clearanee
d. 2 Pieces 1x1 colored lD without headgear for identification card
e. Recent or current payslip
Service Record, and
g. Photocopy of Appointment

5. Above-named BFP personnel are hereby relieved from their unit of assignment and
placed under Detailed Service (DS) forthe duration of FBRC training.
6. Said order shall be automatically terminated upon completion of the said course
unless sooner terminated by proper authorities and participants are advised to report back to
their unit of assignment upon termination of Detailed Service.
