Our preschool football program has a
mission of making each child's first sports
experience a positive one, building
confidence in their social and athletic
skills through a team setting.
Your child will not only learn the basic
skills needed to play football, we will also
help him/her develop other skills needed
to play team sports as they get older. We
focus on listening skills, character
development, encouragement, and
teamwork. Camp Moats Academy is the
perfect program to introduce your child
to sports and foster their love of sports
for life.

WAIVER & RELEASE:
Regardless of the care taken to avoid injury, sports participation can be inherently
dangerous , making it impossible to ensure the safety of all participating individuals.
Although many risk can be avoided, some simply cannot. I release Camp Moats
Academy and the above mentioned learning facility from responsibility for any injury
that my child may incur during the course of the class. In the event of a medical
emergency, Camp Moats Academy will call EMS to render assistance and I will be
financially responsible for any expenses involved.
I certify that the above-mentioned participant is physically fit to safely participate in
Camp Moats Academy, and has indicated any health-related conditions in the Special
Needs portion of the application. I do hereby consent and agree that Camp Moats
Academy has the rights to take photographs of my child and use those images on the
Camp Moats Academy website & future brochures. My child’s name will not be
revealed.

POLICIES & PROCEDURES
Please note there is a $25.00 returned check fee. During inclement weather we will
use an indoor space when available. In the event the weather is too hot, cold, or
raining there will be a maximum of two make-up weeks after each session. Camp
Moats academy reserves the right to cancel any session due to insufficient enrollment.
A full refund will be given if the session is canceled. A cancellation requires a two
week written notice emailed to [email protected] Upon cancellation, a refund
for any remaining classes will be made. Refunds are not given for non-attendance at a
scheduled class. There are no make-ups if a class falls on a National Holiday.
