It is with great pleasure and pride that we welcome you to join CSPD’s 40th
and WSPD’s 13th Annual Meeting in Dana Point. We are excited to once
again present a lineup of renowned speakers for our scientific sessions and
provide an atmosphere that carries you away from the busy city life.
Our program will be of interest to all; from the graduate resident students
and recent graduates to the seasoned veterans that can see the light at
the end of the tunnel. We have outstanding session topics on the latest
technology and materials as well as the business side of pediatric dentistry.
We have added courses for your auxiliary staff to help make your practices
run smoother. As you can see, our scientific program has something for
everyone.

ANITA YORK is a
partner with the
law firm of Scott
& Whitehead
and devotes her
practice to the
representation of
employers, including
more than 100 dental practices. Ms.
York’s counseling practice includes
advising dentists and other employers on
a wide range of personnel issues, policies
and practices, as well as the review and
preparation of employee handbooks.
Ms. York also represents employers and
managers in defending against legal claims
brought by former (and current) employees
in a variety of areas, such as wage and hour
(including “Labor Board” claims), wrongful
termination, discrimination, harassment
and other legal disputes.
ARIANE TERLET, DDS
is a 1986 graduate of
the University of the
Pacific School of
Dentistry. She has
a private practice
in Berkeley, CA
and is the Chief Dental
Officer for La Clinica de La Raza a Federally
Qualified Health Center located in
Oakland, CA. Dr. Terlet oversees 11 clinics
in Alameda, Contra Costa and Solano
Counties and is responsible for program
development and quality assurance. Dr.
Terlet served on the California Dental
Board. She chaired committees on
legislation, continuing education and the ad
hoc Committee on Foreign Schools.
Dr. Terlet has served on the Government
Affairs Council for CDA ,Work Force
Committees, CVP (Committee on
Volunteer Placement) and is an ADA and
CDA delegate. She is a Trustee for the
California Dental Association representing
the Berkeley Dental Society. She is also a
Past President of the Arthur A. Dugoni
School of Dentistry Alumni Association. Dr.
Terlet is a member for the American College

of Dentistry, The Pierre Fauchard Academy
and the International College of Dentists.
Dr. Terlet is very involved in Access to Care
issues at both the State and National levels.
She is a member of the Oral Health Access
Council (OHAC), the National Network for
Oral Health Access (NNOHA) and a board
member for the Center for Oral Health. Dr.
Terlet is married and has three children ages
27, 25 and 17.
DEBR A QUARLES is
a positive, focused
and motivated
professional with
over 30 years of
experience in the
dental field. She has a
unique ability to assess
dental practice productivity and a keen
talent for communicating. She has worked
with Salt Dental Practice Management
for more than 15 years and has helped
hundreds of dental professionals remember
why they love their work.
ELIZ ABETH SNOW
has an over 20 year
background in
health care policy,
legislative advocacy,
political activity
and association
management. She has
worked in government affairs in various
capacities as a contract lobbyist for a variety
of health care interests and on staff with the
California Optometric Association and the
California Dental Association.
After many years with the California Dental
Association, she left in 2009 to become
CEO of the California Building Industry
Association. Joining the association during
the height of the recession and a time that
brought devastation to the home building
industry, she worked to restructure the
organization both at a staff and membership
level. After completing the restructure and
developing a transition plan, she rejoined
the California Dental Association as Chief
Operating Officer.
After a rewarding career focused in
association management, she realized her
heart was still in the policy and advocacy
arena. When a long-time dentist colleague
and friend was elected to the California
State Assembly, she decided to make a
significant career shift and go to work as
a legislative staff person. She currently
serves as the Chief of Staff for Assembly
member Jim Wood who represents the 2nd
Assembly District, which covers the North
Coast of California (northern and coastal
Sonoma, Mendocino, Humboldt, Del Norte
and Trinity Counties)
She has served on a number of boards
including Californians Allied for Patient
Protection, Mercy Prenatal Care,
Californians for Progress and Education,
Center for Oral Health and the California
Dental Association Foundation.
Ms. Snow has a Bachelor’s degree in
political science and is in the process
of obtaining recognition as a Certified
Association Executive.
JARED FINE DDS, MPH
was the Dental Health
Administrator
for the Alameda
County Health Care
Services Agency,
Public Health
Department for
nearly 39 years where
he has been a leader in the development of
innovative delivery systems and programs
that promote the health of children and
families emphasizing access to primary
prevention and treatment. He established
the first continuously operating school
based/ linked dental sealant program in
1993 in two Oakland elementary schools.
He has piloted a successful national
demonstration program known as Healthy
Kids Healthy Teeth, to increase access to
dental care for Medi-Cal eligible children
up to five yours old years. Dr. Fine was
Chair of the Scientific Advisory Committee
of the California wide First Five Oral Health
Initiative and co designed the continuing
education program on early childhood
caries prevention for the dental and medical
community. He established a collaborative
program of primary preventive services
for children at WIC sites operational
since 2008. He led Alameda County to the
development of a five year strategic plan
for oral health in 2012. Dr. Fine was an
inaugural fellow in the USPHS Primary
Care Policy Fellowship, has served as
President of the Alameda County Dental
Society, Chair of the Oral Health Section of
the American Public Health Association,
Chairman of Board of the Dental Health
Foundation. He recently served on the
California Dental Association Taskforce on
Workforce and Access to Care. He was the
2008 recipient of the Myron Allukian Jr.
Award from the Association of Community
Dental Programs and the 2013 recipient
of the John W. Knutson Award in Dental
Public Health from the American Public
Health Association.
JEFF LAABS, RCP has
been a Respiratory
Care Practitioner
for 30+ years. Jeff
is currently the
Chairman of the
Regional Emergency
Cardiovascular Care
Committee (ECC) for greater San Diego,
including Orange County, Long Beach,
Riverside, San Bernardino, Imperial,
San Diego and Desert regions. He is the
immediate past Chair of the Orange
County and Long Beach ECC Working
Groups along with Chair of the QA/QI
committee. Jeff provides all life support
training at University of California Irvine,
Medical Center, the Weil Institute of critical
Care Medicine (WICCM) and other large
institutions. He also provides lectures on
resuscitation at major critical care, nursing
and respiratory care conferences. He is the
founder of RCP Advanced Life Support an
AHA training center providing education
for more the 10,000 people annually. Jeff
will be teaching at the WICCM, 52nd
Annual Critical Care Symposium in April
of 2015. He is the project lead on Ross’
stores “CPR in Schools” program providing
CPR training for all 7th and 8th graders
in California. Jeff recently attended and
completed the inaugural “resuscitation
Training Officer” program from the AHA.
JOHN FEHMER is a 28year dental practice
management
veteran and has
worked with some
of the country’s
largest group
practices. Since 1998,
he has served as Chief Executive Officer
of Children’s Dental Group, founded by
Dr. Scott Jacks. Together, John and Scott
developed a network of nine pediatric
practices in California, including two
locations in Santa Clara County that are the
result of a joint venture with a local public
health non-profit. The enterprise treats
over 100,000 children annually and ranks
among the larger pediatric provider groups
in the Denti-Cal program.
KEVIN D ONLY, DDS,
MS is currently a
Professor, Chair
and Postdoctoral
Program Director
in the Department
of Developmental
Dentistry and Professor
in the Department of Pediatrics at the
University of Texas Health Science Center
at San Antonio. Previous positions include
Professor and Associate Director of the
Center for Clinical Studies at the University
of Iowa, Associate Professor, Pediatric
Dentistry at the University of Texas Dental
Branch at Houston and Associate Professor,
The University of Texas Medical School.
He received his DDS in 1984, Certificate in
Pediatric Dentistry in 1986, and MS in 1986
from the University of Iowa.
Dr. Donly is a Diplomate of the American
Board of Pediatric Dentistry, is a past
President of the American Society of
Dentistry for Children and past Chair
of the Public Information Committee
for the American Academy of Pediatric
Dentistry. He served on the Board of
the American Academy of Pediatric
Dentistry Foundation, the American
Dental Association Foundation and was the
American Academy of Pediatric Dentistry
Academic Trustee-at-Large. He is the
Director of the American Academy of
Pediatric Dentistry Comprehensive Review
Course. He has published over 300 chapters,
manuscripts and abstracts associated with
pediatric dentistry and dental restorative
materials research and clinical utilization.
Presently, he is a principal investigator on
a grant sponsored by the National Institute
of Dental and Craniofacial Research. He
has received grants or research support
from the 3M, ESPE, Premier, Bisco, GC,
Dentsply, Ivoclar, Kerr, Procter and Gamble,
Church and Dwight, Optiva, Oral-B,
Enamelon, Atrix Laboratories, Inc. and
Guidor companies.

LINDA ANN
WAGGONER, MFT is
a licensed marriage
and family therapist
(MFT) in the
state of Nevada.
Additionally, she is
pursuing her licensure
for drug and alcohol counseling. Beginning
her professional career as a registered nurse,
Linda Ann stepped out of the professional
workplace for 25 years and into her role as a
full time mom and wife, her favorite jobs of
all time! After launching her four children,
she returned to school to earn her MFT
degree and has been in the counselling
field for over eight years. She utilizes
Emotionally Focused Therapy (EFT) in her
counseling sessions. EFT focuses on the
underlying emotions that drive behavior.
Understanding behaviors becomes clearer
when we know and understand the
emotions driving the behavior.
Linda Ann has been married to her
husband, Bill for over 36 years. They
have raised one daughter and three sons.
The stay-at-home house manager phase
of her life has given her many practical
experiences in dealing with married
life, toddlerhood, elementary aged kids,
teenagers and, now, adult children. She can
now draw on all of these life experiences to
help marriages and families thrive. In her
spare time, she enjoys reading, visiting her
adult children, investing further into her
professional development and acting as a
leader and mentor for college age young
men and women.
RICHARD MUNGO, DDS
is a private practice
pediatric dentist in
Huntington Beach,
CA. His teaching
responsibilities
include clinical
teaching on craniofacial
anomalies/cleft palate
therapy, lasers in dentistry, dentistry for
the handicapped, and hospital dentistry at

USC School of Dentistry, Department of
Pediatric Dentistry and as a guest lecturer
in the Department of Hospital Dentistry,
UCLA School of Dentistry.
He received his DDS degree from Case
Western Reserve School of Dentistry,
and completed his Pediatric Dentistry
Residency at Boston University. He also
received a Masters in Medical Education
from the USC School of Medicine.
Dr. Mungo serves as Medical Director and
Immediate Past President of the Board for
the nonprofit Healthy Smiles for Kids of
Orange County. He was recently recognized
by California Dental Association with the
Distinguished Service Award in 2008 for his
dedication to the oral health of the children
of California, as well as the Distinguished
Alumni Award for Community Service from
the Boston University School of Graduate

WILLIAM WAGGONER
DDS, MS has been in
full-time practice of
pediatric dentistry
in Las Vegas, NV
since 1996. He
graduated from the
West Virginia University
College of Dentistry in 1980 and completed
his Master of Science degree in Pediatric
Dentistry at the University of Iowa in
1982. From 1982-1986, Dr. Waggoner
held a full time academic appointment
at the University of Oklahoma College
of Dentistry. From 1986-1995 he was
Associate Professor of Pediatric Dentistry
and Director of the Children’s Dental
Clinic at the Ohio State University College
of Dentistry. Dr. Waggoner has numerous
research publications in peer-reviewed
dental journals and is the author of several
chapters in pediatric dental textbooks. He
is a Diplomate of the American Board of
Pediatric Dentistry and a Fellow of the
American Academy of Pediatric Dentistry.
Dr. Waggoner is a frequent speaker on
the subject of dentistry for children. He
has been a featured seminar speaker of
the American Dental Association and has
given dental presentations internationally
in areas such as Russia, Portugal, China,
Korea, Thailand and Argentina and in over
30 states. He is a member of the American
Dental Association, American College of
Dentists, American Academy of Pediatric
Dentistry, and is past President of the
Nevada Academy of Pediatric Dentistry. He
currently owns and operates two pediatric
dental offices in Las Vegas with nearly
30 employees and employees three other
dentists.
Dr. Waggoner has been married for 35
years to his wife, Linda Ann, a marriage
and family therapist. They have four grown
children. His spare time is devoted to family
activities, reading and traveling.

numbers who have received care . Best estimate as to the
number of providers (General Dentists, Pediatric Dentists)

the reasons behind the introduction of legislation which
supports the concept of Mid-level providers as a possible
solution to the access problem and how the pressure to adopt
this solution will not go away soon. He will also discuss the
effects of the Affordable Care Act on numbers of patients
covered by Medicaid in future years which will only add to the
need to find innovative solutions to the problem.

In 1950, Humphrey introduced the stainless steel crown to
dentistry as a method of restoring badly decayed primary
molar teeth. This has remained a mainstay of restorative
dentistry for pediatric dentists for over 60 years. However,
although the durability of stainless steel crowns continues
to be superior to almost every other pediatric restorative
option, many parents are requesting and demanding more
aesthetic restorations for their child’s teeth. This presentation
will discuss aesthetic full coverage options for badly decayed
primary teeth with an emphasis on pre-made zirconia
primary crowns, pre-veneered primary crowns, and full resin
based composite crowns. Indications and advantages and
disadvantages of each type of crown will be discussed as well
as tips on preparation, fitting and cementation. Information
from the recent AAPD Restorative Symposium will also be
included.

An Update on Restorative Dental Care for Children

demineralization. Intervention with fluoride (including
fluoride varnish), glass ionomer surface protectants,
sealants and minimally invasive products and procedures
will be presented. Risk assessment utilization in decisionmaking for restorative care will be discussed and related
to recommendations made by the American Academy of
Pediatric Dentistry for restorative dentistry care in children.
Course Objectives:
• Be exposed to early diagnosis of enamel demineralization
and understand preventive regimens available, as well
