 Non-profit organization
 Job Function: Conduct engagement services within the Care Coordination (CC) Department including
eligibility screening and intake activities as well as provide care management services to individuals
living with HIV/AIDS and/or other chronic illness that live in the Finger Lakes and Southern Tier
Region.
 Hours: Full Time Position -- Primarily Monday -- Friday, 8:30 am -- 4:30 pm. Hours may vary based on

 Specific Duties: Provide direct care management services maintaining accurate and verifiable
documentation of all services rendered. Complete comprehensive and detailed intakes, assessments,
care plans and any other paperwork as required by standards set by DOH and lead Health Home
contractors. Assist clients with applications and paperwork for and management of benefits, financial,
housing, or other related areas as needed. Provide assistance to clients in accessing community-based
supports. Provide crisis intervention and follow-up. Other duties as outlined in Position Description.
 Qualifications: Bachelor's degree in Human Services or related field and one years of care
management or case work with persons with disabilities and/or chronic illnesses. OR
