Toronto Research Chemicals ‐ Weizmann Canada Award for Scientific Achievement Sponsor: Toronto Research Chemicals and Weizmann Canada Canada‐Wide Science Fair Permission Form for the International Trip to the ISSI in Rehovot, Israel. This scholarship provides the opportunity to attend the Dr. Bessie Lawrence International Summer Science Institute (ISSI) at the Weizmann Institute in Rehovot, Israel from July 1st to July 29th, 2015. The winner must have a single project, demonstrate a passion for science, have graduated from high school or Cégep prior to the starting date, and be fluent in English. The total scholarship value is about $5 000, which pays for the registration fee and the return air fare from a major Canadian airport to Israel. This permission slip ensures that you are aware of the trip, and the responsibilities this has for both the child and parent/guardian. Please fill in the check boxes, and then sign the form. It must then be uploaded to the CWSF registration system as part of the application for this award. This is a project carried out by a single student. The student is fluent in English. The applicant will have graduated from High School or Cégep prior to the trip. I will pay for the cost of the travel to and from a major Canadian airport. I will pay for incidental expenses.
