Individuals interested in participating in the CWTC Bowl-A-Thon should complete and return this
form. If you wish to enter a team of four, please include the names of the individuals
on the team in the space provided. Only one form is required per team.
*The cost for individuals receiving services from CWTC is $7.00 per person.
