A Note from the President
New Providers

New Vice President of Business Development
Our company is poised for great opportunities in the
coming months and years ahead. We have been
through a significant amount of change, and we have
all learned a great deal from each other, our patients
and our long term care partners. We have added some
“top notch” outside help during this transition time in
Mark Hunt and Chris Johnson. I take great pleasure in
announcing that El Harris joined the Five Star team as
the Vice President of Business Development. El was
formerly the President of PrevMED, a Dental Case
Management company serving the long term care
communities in six states including Maryland. Nine
years ago, El was one of PrevMED’s first employees,
serving as a Business Development Executive, and
helped to grow their business to serve almost 400
nursing homes with annual revenues of $18M. His
entire career has been focused on business development
in both the I.T. and Long Term Care communities.
El’s primary focus at Five Star will be to work with
each of you to build existing facility relationships, as
well as grow our services into new long term care
communities. You can expect to hear from El over the
next few months as he gets to know the most important
part of Five Star, you. Please join me in welcoming El
to the team.
New Providers to Serve Patients and Clients

Dr. Otashe Golden joins Five Star from private practice
in California. She received her Medical Diploma and
training at the University of California, Davis. She will
begin treating patients at our Virginia facilities and
Future Care Pineview beginning July 1, 2015.
Dr. James Petit began seeing patients at Lorien Bulle
Rock on March 9, 2015. He was a Hospitalist at Sinai
Hospital, Baltimore. He graduated with his Medical
Diploma from Howard University and completed his
residencies at Howard University and Sinai Hospital.
Dr. Edgar Potter has served as the President of Ft.
Washington Health and Rehabilitation Center in Fort
Washington, MD. He received his Medical Diploma and
training from Howard University. He began treating
patients for Five Star at Ft. Washington on April 1,
2015.
Dr. Sibte Kazmi has been in private practice since
2003, as well as serving as the past Medical Director for
Golden Living Center in Frederick, MD. He received
his Medical Diploma from the University of Karachi,
Pakistan and completed residencies in the United States
in Internal Medicine, Clinical and Anatomic Pathology.
He will begin treating patients in our Five Star Frederick
facilities in May 2015.
Dr. Nader Tavakoli is now the Medical Director at
Baywoods of Annapolis and Ginger Cove for Five Star
since early 2015. He received his Medical Diploma
from Ross University School of Medicine and
completed his training at Prince George’s Hospital
Center.
Esther Schaftel, CRNP, a Nurse Practitioner for 12
years, has joined our practice after working in long term
care, orthopedic surgery and physical medicine and
rehabilitation. She graduated from the University of
Maryland.
