Midhurst Mile & Bike Rodeo BBQ - Thursday, May 28th, 2015 ~ 5:00-7:30 p.m.
Our Midhurst Mile & Bike Rodeo is a volunteer run event to help raise funds for an Elkay Water Bottle Filling
Station at Forest Hill Public School. Come out and celebrate our school spirit with classmates, family and friends!
This year we will be offering Gourmet Hot Dogs, Fries, Sliders, Beaver Tails, Soft Drinks, Water and Freezies!
Parents are asked to please pre-pay for the selections below (CHEQUES ONLY, THANK YOU, payable to Forest
Hill Public School). Please fill out the pre-order form, and, return it to the school by Friday, May 22nd using one
form per family. While quantities last, a limited supply of items will be available to purchase the evening of the
event, as pre-orders will be served first.

What is the Midhurst Mile? Since 2007, Forest Hill has been raising funds for the maintenance of our
school track and student athletics through this spirited celebration!! This year funds will go toward an
Elkay Water Bottle Filling Station . Come run or walk the mile!
The race route is a one-mile loop through our community – starting and finishing at the Rob Plunkett Memorial
Track. We’ll have a “race bunny” you can try to catch!
This annual event is fun for the whole family! Registration begins at 5:00 p.m. Please check in at the registration
desk to pick up your pre-ordered food vouchers & wrist bands to participate**
While there is NO FEE to participate, Donations are GLADLY accepted and any donations in excess of
$20.00 will be issued a tax receipt!
Warm up with Miss Connie at 5:45 p.m. The race will start promptly at 6 pm.
Enjoy dinner from Michael & Marions and a delicious Beaver Tail treat! Please pre-order! Only limited sales will
be available the night of.
Our Bike Rodeo, organized by the Springwater Policing Committee, will be fun and educational for all ages! Come
and test your bike safety knowledge and practice your skills in the Scooter Zone or Skateboard Park!
Certificates will be given to all participants.
Rain or Shine ~ Come Celebrate “Spring” with your Forest Hill friends, family and Sparky the Fire Dog! There will
be a booth set up by The Springwater Firefighters for fire prevention education.
The class with the most participants at the Midhurst Mile will receive an ICE CREAM PARTY for the entire
class!
Some of our partners for this event include Springwater Policing Committee, Springwater Fire Fighters
Association, OPP, Cycle Simcoe, Hardwood Ski & Bike, Bikeland, MEC Barrie, Joe’s No Frills, Michael & Marions,
Beavertails, Wendy’s and a number of other local vendors.
** There will be a waiver that will require a signature to participate. If your child would like to participate,
please complete the attached liability waiver and return it to the school along with the food order form by
Wednesday, May 22nd. Also available on the Forest Hill P.S. website.
The first 100 registrants will receive a loot bag!!!

FOREST HILL 2015 WAIVER, RELEASE AND INDEMNIFICATION In consideration for permission from Forest Hill Public School (FHPS) and Partners to participate in Forest Hill Midhurst Mile/Bike Rodeo on May 28,2015 (the “Activity”), and in full recognition and appreciation of the dangers and risks inherent in the Activity, I hereby waive, release, and forever discharge FHPS and Partners(and all of its affiliates, elected officials, directors, officers, employees, contractors, agents, volunteers and those for whom FHPS and Partners is in law responsible) (collectively, the “Releasees”) from and against any and all liability, claims, demands, actions, or causes of action for costs, expenses or damages to property or personal injury (including death) (collectively, “Liability”), which may result from my or my child’s participation in the Activity due to any cause whatsoever, including without limitation, negligence, or breach of any statutory or other duty of care on the part of FHPS. I further hereby agree to hold harmless and indemnify the Releasees from and against any and all Liability which may result from, or is in any way connected with, my or my child’s participation in the Activity. Partners include Township of Springwater, Hardwood Ski and Bike, Bikeland, MEC Barrie, Cycle Simcoe ACKNOWLEDGEMENT OF RISKS I understand and acknowledge that participation in the Activity is voluntary. I understand and acknowledge that the Activity may be hazardous, that participation is solely at my own risk, and that I assume full responsibility for any injuries (including death) or damages resulting from my or my child’s participation in the Activity. I affirm that I am, and/or my child is, in good health and at an appropriate level of physical fitness and physical capability to participate in the Activity. I affirm that I am at least 18 years of age, or that I am the parent and/or legal guardian of the child listed below and I hereby consent to their participation and waive liability on their behalf. CONSENT TO USE IMAGE I hereby consent and authorize FHPS and Partners to take my photograph/image and/or the photograph/image of the child listed below, for whom I am the parent and/or legal guardian (collectively, “Image”). I acknowledge and agree that FHPS and Partners may publish, display or use the Image for any FHPS/Partners purpose and by any means whatsoever including, but not limited to, electronic or digital means. I acknowledge that FHPS and Partners may not be able to control the distribution or use of the Image by other than FHPS/Partners representatives. I agree that this consent is given in perpetuity and for no consideration, credit, acknowledgement or financial recompense, now and in the future. I hereby waive, release, and forever discharge the Releasees from and against any and all Liability whatsoever which may result from the taking, use, display, publication or distribution of the Image. BY SIGNING THIS DOCUMENT, THE PARTICIPANT / GUARDIAN ACKNOWLEDGES HAVING READ THIS DOCUMENT AND UNDERSTANDS THAT THE PARTICIPANT / GUARDIAN IS WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.
