Look for every opportunity to find sponsors to pledge money! Every bowler must bring in a minimum of $50 in pledges,
including your own donation, if desired. Please total your pledges and bring all pledges with you on event night. Your
sponsors will receive receipts for any donation exceeding $250 (receipts for donations under $250 upon request). Checks should

Each bowler must complete and submit this form. Whenever
possible, bowlers forming a team should reserve their lanes at
the same time as their teammates (up to 6 bowlers per lane).
Every effort will be made to assign bowlers with their
teammates. To assist us in assigning team members to lanes,
please include all your team member names on this form.
Reserve your lane early – space is limited! Please print
clearly. Please note that we cannot guarantee that late
additions to teams will bowl on the same lane as their
teammates. Children ages 12-17 can be a part of you bowling
team for $25.00. Bring a minimum of $50 in pledges
(including your own donation, if desired) on event night.

Second Chance at Life (SCAL) is a 501(c)(3) nonprofit that provides
financial support to transplant patients in Michigan who cannot
otherwise afford prescriptions, medical bills, and other necessities
during and/or after a transplant. SCAL is committed to focusing on
being “Patient Centered and Patient Driven”. Since our inception,
SCAL has granted more than $250,000 to transplant patients and
participating transplant hospitals in Michigan.
