I agree to provide truthful and complete information. I understand that Grandview High School Administration has the
right to verify this information. I also understand that Grandview High School Administration has the right to refuse
admittance of my guest to the dance. I may only register and bring one guest.
The following rules apply:
• I must register my guest by the due date of Monday, April 13th in the activities office.
• My guest is my responsibility. If they are asked to leave, I will also be asked to leave.
• I will not be able to buy a ticket until this guest registration contract is completed, signed & approved by Admin.
• School issued ID is required at the door. Driver’s license required for those not in high school.

*The Parents/Guardian of students who would like to bring a guest not attending high school must sign
below the black line (and provide the phone numbers requested). The student and his/her parent will
then assume full responsibility for this non-high school guest. If there are any concerns by the GHS
administration, further steps for approval may be taken. Non-high school guests will not be admitted
unless this process has been fully completed by Monday, April 13th. The non-high school guest will need
to present their driver’s license or state issued ID at the dance or they will not be admitted.
