RELEASE STATEMENT
For and in consideration of participation by my child in the Holy
Cross Basketball and Volleyball summer camps, I agree to hold Holy
Cross and its employees harmless and to waive the right to bring legal
action against Holy Cross High School and its employees for any injuries
sustained during the course of its sports camp.
Participants must carry their own accident and/or medical insurance.
Coaches and instructors of the Holy Cross summer camps are safety
conscious and follow appropriate safety procedures. In the event of injury
or illness, every effort will be made to contact the parents or guardians.
I authorize Holy Cross summer coaching staff to administer first aid
and to call first responders in the event of serious injury. The participant
has had a medical examination within the last 12 months, and by my
signature I certify that my child has no pre-existing condition that would
prevent his/her full participation in the sports camps.
This agreement and waiver, having been read thoroughly and
understood completely, is signed voluntarily on the registration form as
to its con-tents and intent.

Purpose
HCHS camps will provide campers the
opportunity to learn fundamentals of the
game and play game situations. Our camp
instructors will consist

will provide an AMAZING experience and
teach what being a competitor is all about.

• Each camper will receive a t-shirt.
• Participants should wear gym shoes only
and should not wear any jewelry.
• Open gym time will be available 30 minutes
before each session.
