AUSTRALIAN EMBASSY, HANOI AUSTRALIAN CONSULATE-GENERAL, HO CHI MINH CITY VISA AND CITIZENSHIP SECTION MEDICAL TREATMENT (SUBCLASS 602) VISA APPLICATION CHECKLIST Please note that this checklist should be used for all Medical Treatment Visa applications lodged by clients in Laos or Vietnam. Important: a. You should complete your Australian medical examination before you lodge this visa. See: https://online.immi.gov.au/lusc/register b. Please note that a separate visa application is required for each person of the travelling party. This subclass does not permit children or other dependent applicants to be included in the same application as their parents. c. Ensure you lodge your visa application at least four (4) weeks prior to your intended travel date. You should ensure that application forms are fully completed and all relevant supporting documentation is provided. While in exceptional cases, an applicant may be requested to provide more information by their case officer, in most cases a decision on a visa application is made solely on the information provided at time of application. d. All foreign language documents submitted with your application must be accompanied by a certified English translation - unless indicated otherwise in the visa application checklist. e. Certified copies need to be authorised or stamped as being true copies of the original document by the local authority in Laos or Vietnam. In Australia, documents may be certified by a person before whom a statutory declaration may be made under the Statutory Declarations Act, 1959. We will not accept photocopies of certified documents. Special Note: Applicants in Laos All visa applications lodged at the Australian Embassy in Vientiane are assessed by the Australian ConsulateGeneral in Ho Chi Minh City, Vietnam. Please ensure you provide certified English translations with all Lao or Hmong language documents. Assessment of your visa will be delayed if you do not provide English translations at time of visa application lodgement. MEDICAL TREATMENT (SUBCLASS 602) VISA APPLICATION CHECKLIST AUSTRALIAN HEALTH REQUIREMENT You will need to meet Australia’s health requirement in order to be granted a medical treatment visa. Please complete your health examination before you lodge this visa application. To do this, you will need to create an ImmiAccount and generate your own Health Assessment Portal (HAP) ID and medical request letter. You will then need to take this letter to a panel medical clinic to complete your health assessment. Create an ImmiAccount: https://online.immi.gov.au/lusc/register Once you have generated your own HAP ID, please let us know what it is. This will make it easier for us to locate your medical examinations once you lodge your visa. Application Fee Check the method(s) of payment accepted for your country of residence: methods of payment Information about your identity One passport size photograph (this photograph must not be more than six months old). Certified copy of your Birth Certificate. Certified copies of your passport bio-data page and each page that has a visa label and tick where include d Official (SDP) use only entry/exit stamps for Australia or another country. Important: - Each page of your passport must be copied onto A4 size paper. - Bring your original passport with you, so we can verify against all certified pages. Vietnamese and Laos national passports will be returned to the applicant after we have processed your visa lodgement. - If you hold a passport from a country other than Vietnam or Laos, your passport will be returned to you once a decision is made on your visa application. Certified copy of your National Identity Card (this document can be in Vietnamese). Certified copy of all pages of the Household Registration Book (this document can be in Vietnamese). Evidence of your current marital status Information relating to your medical treatment Details of the referral source: Doctor’s name, name of clinic, address of clinic, telephone number and email address. Details of the medical practitioner/facility in Australia where you will receive treatment: Doctor’s name, name of clinic, address of clinic, telephone number and email address. Written confirmation from the medical practitioner/facility, on official letterhead that: they agree to treat the patient; the nature and estimated duration of the treatment; proposed dates of treatment and/or consultation; a proposed date of admission to the relevant medical facility If the treatment is to be provided by a private hospital or private medical practitioner (including a private medical practitioner in a public hospital), written confirmation from the private hospital/practitioner, on official letterhead that: they undertake to treat the applicant; and they are satisfied as to the arrangements for the payment of all costs related to the treatment (including fees for consultation, treatment and hospital accommodation); or the relevant hospital, medical practitioner and/or other health care providers have agreed to give their services free If the treatment is to be provided by a public hospital/clinic in Australia, written confirmation from the relevant Australian State or Territory Health Authority that: the authority undertakes to treat the applicant in that public hospital; and no Australian will be disadvantaged the authority is satisfied as to the arrangements for the payment of all costs related to the treatment (including fees for consultation, treatment and hospital accommodation). Additional Information required for children under 18 years of age If the visa applicant is under 18 years of age, and intends on travelling without one or both their parent(s) or legal guardian(s) please provide the following: A completed Form 1229 signed by any non-accompanying parent(s) or legal guardian(s); or An original letter signed by any non-accompanying parent(s) or legal guardian(s) consenting to the grant of the visa. This letter must clearly state the child’s name, name of the consenting parent(s)/guardian(s), and consent for the child to travel to Australia with the person accompanying them; or A court order from Australia, Laos or Vietnam permitting the removal of the child. Please note that Form 1229 or the letter must be certified by the local authorities.
