this packet. You must attend Prom in order to attend Post Prom.
COAT CHECK:
There will be no coat check available at Prom. Please make your own arrangements to keep your valuables secure. Do
not bring valuables or an excessive amount of money with you to the Prom.
PARKING & TRANSPORTATION: Parking at Belvedere Banquet Hall is limited. It is recommended that students consider carpooling
and/or reserving limousine service or arranging to be dropped off at the Belvedere.
PROM COURT:
Elections for the Junior and Senior Prom Court, Prom King and Prom Queen will be held during Prom
ticket sales.

Prom is an official school event. All District 211 rules and regulations apply.
The use or possession of alcohol or any controlled substance before or during the
Prom will result in exclusion from that event and disciplinary action.

The NO SMOKING policy applies to all students and guests.
Anyone leaving Belvedere Banquet Hall MAY NOT RETURN.
No students and/or guests will be allowed to reserve rooms
at the adjacent hotel for any reason.

2015 HEHS POST PROM EVENT
INFORMATION SHEET
**PLEASE READ BOTH PAGES CAREFULLY**
Spring is in the air! Soon the H.E.L.P. Board will proudly sponsor our drug, alcohol and smoke free 2015 HEHS Post
Prom Spirit Boat Cruise on beautiful Lake Michigan. Tickets may be purchased by any Junior or Senior attending the
Prom.
Please note the following important information:
Post Prom Event Information:
Date: Saturday May 30, 2015 (immediately following the Prom)
Time: 11:15 p.m. to 3:45 a.m. (approximately)
Place:Buses will leave the Belvedere Banquets immediately following Prom (approximately 11:15). We will return to
the Belvedere Banquets in Elk Grove around 3:45 a.m. All students MUST travel to and from the event in H.E.L.P.
provided transportation.
Ticket Sales Information:
Cost: $50.00 per couple, $25.00 single.
Cash only! No checks will be accepted. No refunds issued.
This price includes transportation, a breakfast buffet, music, dancing and a beautiful view of the Chicago Skyline!
Thanks to H.E.L.P.(Hoffman Estates Loyal Parents), the Post Prom Committee fundraising, and donations from
outside community businesses, additional funds were raised that helped lower the cost of this event to make it more
affordable for students. Without these efforts, each ticket would have cost $130! This is an example of how your
H.E.L.P. membership dollars are being put to good use for our students.
Dates Sold: Wednesday, May 6th from 6:00 p.m. – 8:00 p.m. at HEHS Thursday, May 7th from 3:00 p.m. – 5:00
p.m. at HEHS
To ensure you are able to be on the same bus as your friends, purchase your tickets early and at the same time.
Procedures: When purchasing your tickets, please be sure to have your Signed permission slip, and your CASH.
Remember the permission slip MUST be completed in full for each person attending the Post Prom, whether they are
an HEHS student or not. If this information is not complete, you WILL NOT BE ABLE TO PURCHASE YOUR
TICKET!
**Please read the important information on following sheet*
Guidelines and Purchasing Information:
" Post Prom is optional, BUT You MUST attend Prom in order to attend Post Prom.
" If you are in a group, and would like to have one person from your group purchase the tickets for your
group, this will be acceptable. That person MUST have all the completed permission slips and cash
from everyone in your group ready at the time of ticket sales, or no tickets will be issued. Also, a
parent may purchase tickets for their child in the event that they cannot be present at the time of
tickets sales, with the same restrictions listed above.
" Post Prom tickets are Non-Transferable and Non-Refundable. If you change your mind and are not going to
use your ticket, please contact a member of the Post Prom committee. Do not give the ticket to anyone else.
This is a safety issue and will be strictly enforced (the ticket is tied to the Emergency form).
" Also, any changes from the original permission slip must be cleared through one of the committee members. For
example if you decide to change your date for the prom, a new permission slip must be filled out before the
day of the prom. Please be considerate and allow us enough time to get this information from you.
" If you wish to sit on the bus with another couple, please be sure to purchase your tickets together. Bus numbers
are assigned in the order that Post Prom tickets are sold.
" The procedure for check-in will be as follows: At the end of the prom, there will be an announcement
instructing you which buses are boarding. You will go to the bus you have been assigned at that time. No one
will be allowed to go to his or her car. You will show your Post Prom ticket to the chaperone assigned to
your bus and they will check your name off the list. Please have your student ID or license (guests must
have ID also) with you just in case there is some reason to verify information. Upon arrival at Navy Pier, the
chaperones will give each student their boarding pass for the Spirit Cruise.
" Remember this is a Drug-Free, Alcohol-Free, and Smoke-Free activity. Please respect these rules.
" No student/guest may provide his or her own transportation to Navy Pier. Everyone must ride TO and
FROM the Post Prom site on the buses.
" All students who purchase Post Prom tickets on the first sale date will be guaranteed a ticket for the Spirit Cruise.
Students who wait to purchase tickets at a later date, may be denied ticket sales if we have reached boat and
bus capacity. Due to the number of students involved in this Post Prom event, it is imperative that the rules be followed so
that we can all enjoy a safe and memorable experience. If any students do not comply with the Post Prom
rules, the Administration will deal with them accordingly. Parents, please discuss this notice with your child. We appreciate your support and cooperation in this
matter. If you have any questions or concerns, please contact one of the H.E.L.P. committee members listed

2015 HOFFMAN ESTATES HIGH SCHOOL POST PROM PERMISSION FORM
My son/daughter has my permission to participate in the 2015 HEHS Post Prom event. I understand that
Student Accident Insurance, if I paid for the coverage, is in force on this trip only when the student is under
the general supervision of a teacher or supervising adult appointed by the school. The student/guest is not
to remove himself/herself from that supervision by unauthorized conduct such as leaving the group.
I further understand that all rules and regulations governing student/guest conduct remain in effect while the
student/guest is participating in this activity. My permission is granted to the supervising adult to allow him/
her to take all necessary actions should an emergency arise. In case of an accident or incident requiring
medical attention, the supervising adult will attempt to contact parents/guardians immediately. Emergency
numbers and physician are listed below. I give the supervisor permission to arrange for medical attention,
and I accept financial responsibility for that attention should the supervisor be unable to reach me.
