Affix a passport size colour photograph in the space assigned. 2. Name of the candidate should be filled in BLOCK LETTERS and leave a block vacant between two parts of the name. e.g. R A V I K A N T S H A R M A 3. Basic Details of the Candidate - Candidate should fill complete information related to his/her parentage, Permanent and corresponding address, contact number, category, and Date of Birth (DD/MM/YYYY) in the space allotted. e.g. 18/04/1990. 4. Academic Details-Candidate should mention his/her academic details of graduation (Appearing / Appeared) in Column No. 9. 5. Candidate should mention the Scores (Composite & Percentile/Percentage) attained in Management Entrance Test (CAT/ MAT/ CMAT/ GMAT/ XAT/ATMA) in Column No.10 and also attach a self-attested copy of the score card along with the form. 6. Written test centres: A written test will be held on 24.05.2015. The candidate may choose two test centres for appearing in IAT 2015. Ref. Column No.11. The tentative list of centers for written test are: Ahemdabad, Bhubaneswar, Bangalore, Bhopal, Noida, Chennai, Goa, Guwhati, Gwalior, Jaipur, Jammu, Kolkata, Lucknow, Mumbai, Nellore, Patna, Srinagar, Tejpur, Trivandrum. 7. Programme Preference-: Candidate should give his/her rank-wise preference for various specialisation of MBA programme in Column No 12. 8. Centre for Group Discussion and Personal Interview(Column No.13)-Candidate should select one centre of his/her choice from the below mentioned centres for appearing in GD & PI, to be held on 20th, 21st, 22nd, and 23rd June 2015. a. In case of the candidates who are appearing in IITTM-Admission Aptitude Test-2015, will submit their Application Form along with application fee, requisite documents and two copies of admit cards. Candidates having scores of CAT/ MAT/ CMAT/ GMAT/ XAT/ATMA will submit their application form along with application fee, score cards & requisite documents. Incomplete application or without application fee and signature of candidate or reaching after last date will not be considered. 10. For detailed information kindly read Admission Bulletin 2015-17 available on www.iittm.net 11. Pattern of IAT 2015: IITTM Admission Test (IAT) 2015 will be conducted on May 24, 2015 (Sunday). IAT will be of 02 hours duration from 10:30 am to 12:30 pm. The candidate has to mark their answers on OMR Sheet. It is an objective type test carrying 100 marks where 01 mark will be awarded for every right answer while there will be no negative mark for wrong answer. IAT includes the following disciplines: i. General Awareness : 50 Marks ii. Verbal ability : 25 Marks iii. Quantitative ability : 25 marks
