communities. Our Health Center is nestled on a gor geous campus in downtown Palo Alto with rose

Admissions
(Admissions available every day of the week - 24/7)
Admission requests, by families, for our Health Center must have prior physician appr oval.
For acceptance we will need:
1. Medical records (history, physical), medical list and progress notes
2. Demographics sheet from physician’s office
3. Physician orders
4. Health Insurance ID card – must be presented within 24hrs of admission (photo copy acceptable)

Explaining Therapy Terms
Occupational Therapy (OT) - Involves rehabilitation with Active Daily Living Skills (ADLS) such as dr essing,
toileting, upper body strengthening
Physical Therapy (PT) - Involves rehabilitation with bed mobility, transfers, ambulation, assisted devices,
upper & lower body strengthening
Speech-Language Pathology (SLP) - Involves maximizing individual's communication and cognitive skills
and also addresses swallowing disorders
-Swallowing deficits and disorders such as dysphagia
-Speech disorders such as aphasia

Our Health Center staffs according to acuity levels, and in all cases complies with staffing r egulations.

The Health Center’s physicians come from Stanford and Palo Alto Medical Foundation and all specialize in
geriatrics.
