You can now apply to KPSAHS programs online! This is an easy way for you to provide all of the information we need about
you, your program(s) of interest, your academic history, documents required for admission, and even a link to pay the
application fee online. KPSAHS no longer accepts paper applications for this program.
Please take time to read these instructions fully as they include specific information regarding the completion of the KPSAHS
Online Application process. If you have questions not covered in the instructions, you can contact the Admissions Office at
(510) 231-5123 for further clarification.
To get started:
• Visit https://www.mykpsahs.com/Applicant/ApplyOnline_Login.aspx.
• If it’s your first visit, click on the Create Account button on the far right of the screen.
o Select KPSAHS Richmond Campus
o Enter your First and Last Name and Email Address.
o You will be given your user name (write it down for future reference) and asked to
create/enter your password and a PIN (any four digit number you will easily
remember).
o You will be taken to the first step of the application process.
• If you’ve already started an application, simply login again using your username and password
previously created, and you’ll be taken to the step in the application process where you left off.
• In Step 1: Enter all required personal information. Click ‘Save and Continue’ after each step.
Personal Statements are not required.
• In Step 2: Enter additional information as required.
• In Step 3: Enter the degree, program and start date in which you are interested. Please also
enter whether you are interested in other program(s) in the ‘Other Programs’ text box.
• In Step 4: Enter your academic history (please see below where to enter Pre-requisite courses).
• In Step 5: A preliminary list of documents that we require to process your application is shown
here. You will submit these by uploading them (via the ‘My Documents’ folder). You will be
required to mail in your officially sealed transcripts (mail transcripts to address below).

information you’ve entered, you may print a copy for your records. Click ‘Submit’.
After your application has been submitted, a message will display which includes a link to the Payment Processing
site. Please use the web payment link provided to pay your application fee.
Be sure to follow the link and pay your fee. Applications will not be considered until the application fee is paid and all
required supporting documentation has been received by the KPSAHS Admissions Office.
Applications with no payment and or supporting documents received by the November 19, 2015 deadline will be
processed as an incomplete application.

Program Overview:
The Kaiser Permanente School of Allied Health Sciences (KPSAHS) Nuclear Medicine Program is 18-months (6 quarters) in
length consisting of classroom and clinical education in Nuclear Medicine. The Nuclear Medicine Technologist uses high-tech
equipment and radioactive tracers to study the function of the various organ systems. This includes imaging the progress of
disease, as well as treatment of disease. The Nuclear Medicine Program is accredited by the Joint Review Committee on
Educational Programs in Nuclear Medicine Technology (JRCNMT).
The Program combines the didactic classroom theoretical concepts with clinical education at various hospitals and clinics
throughout Northern California to full integrate the student’s experience in the excited field of Nuclear Medicine Technology.
Online Application Procedure - Please refer to the Application Instructions [pdf] located on the KPSAHS Nuclear
Medicine Program web page where the Application Procedure outlined below is explained in detail.
1. Online Application – Applicants must enter in all required information through the online application portal
in order for your application to be reviewed.
A completed application includes the following:
• Completed online application, submitted and verified no later than the specified deadline.
• Two (2) letters of reference bearing an original signature and current date (must be dated within 6
months of application submission date). The letters should be from persons who can address your
competencies, abilities, experience and character related to your academic, professional, and/or
employment activities. Uploaded via application portal.
• To verify completion of prerequisites, all educational information provided must be validated by
officially sealed (unopened) academic transcripts and accompany the application packet. While
KPSAHS recognizes coursework from Regional Institutional Accrediting Organizations and
Professional Accrediting Organizations, KPSAHS does not recognize all Professionally
Accredited Organizations. See Application Instructions for submission of Military and Foreign

$55.00 non-refundable handling fee for each program. (Paid via PayPal through the application
portal) Must be paid at the time of submitting your online application.
Pre-requisite Course Evaluation – All required pre-requisite courses entered through the application portal
will be verified by admissions upon the submission of your officially sealed transcripts.
All required program pre-requisite coursework completed at previously attended regionally accredited colleges and
universities will be evaluated and posted to your KPSAHS application as part of the admissions process. The prerequisite evaluation will be done upon receipt of your complete submitted officially sealed transcript(s). The
Admissions Office evaluates only official copies of transcripts after an application for admission has been received.
Course work must be college level, completed and documented on an officially sealed transcript and consist of a
minimum of 3 semester units or 4.0 quarter units with a grade of “C” or higher. Do not list “in progress” courses.
KPSAHS does not accept Pass/Fail, Credit /No Credit courses. Due to the large number of applicants who
apply for admission, we are not able to provide "on-the-spot" evaluations.

3. Interview Process – Upon the completion of the Application Evaluation process. Those applicants who are
selected for an interview will be contacted by phone. Due to the large quantity of applications received,
KPSAHS does not obligate itself to interview all applicants.

4. BS Admission Requirements - Applicants for the Bachelor of Science degree options must satisfy the following
requirements for Admission:

Submit official transcripts from a USDE-recognized regional accredited institution, to show completion of a minimum
of an AA/AS degree and a cumulative minimum grade point average of 2.75, with verification of satisfaction of lower
division California Community College/California State University (a) to (e) pattern required as a condition of
admission.
•
A minimum of (9) Semester Credits or (12) Quarter Credits ( 3 courses) meeting the following expectations:
a) 3 Semester Credits or 4 Quarter Credits minimum credit per course
b) Upper Division General Education credit from awarding institution; must meet the KPSAHS General Education
Learning Outcomes
c) Minimum 2.0 GPA earned per course;
d) Academic credit from a regionally accredited institution,
e) Coursework from the subjects of: Ethics; Diversity; Scientific inquiry/Research Methods; or Business
Administration (Scientific Inquiry required, with remaining credit from a minimum of two of the Elective
options); and Comparable to Learning Outcomes as established by KPSAHS.
f) Upper Division coursework completed within 7 years
g) Coursework must confirm completion of lower division general education requirements to meet California
Community College/California State University requirements of the (a)-(e) requirements.
