There will be no later notice; your son will be registered upon receipt of this form.
Discounts Available: A $5 discount (up to $10 total per camper) is available for campers who attend more than one
camp. Just subtract $5 from each additional camp – up to $10 total per camper. Two camps = $5 off, three camps = $10 off.

***** MBBC will reimburse 50% of camp costs ($200 maximum total for all summer camps) for members only. The
camper’s parents must request the reimbursement with the club.
Liability Release Form: As parents (guardians) of the above child, I hereby give permission for my son to participate in Mandan Summer
Basketball Programs. I release the sponsors, camp staff, director, Mandan Public Schools, and the Mandan Park Board from any liability or theft
that may occur to my child while in attendance.
