Checklist to Apply:
Please Submit Three (3) 4x6 photos of your
work, please include a photo of your workspace
and/or a photo of you creating your work.
(Photos will not be returned) **New Applicants
Only** (unless your product has changed).
On a separate sheet of paper, describe your
work. NO RESALE ITEMS ALLOWED. Tell us
how your work stands out from the rest, what
materials you use, how you create and come up
with ideas for your handcrafted items or anything else you feel we should know about your
crafts. **New Applicants Only**
Exhibited work must be consistent with the
photos submitted to the jury
You are responsible for collecting 6.875% Minnesota Sales Tax from the show. If you do not
have a Minnesota state ID call (651) 282-5225.
Enclosed is a copy of an ST19. Please complete
and return with your registration form.
Include Payment to reserve your booth space.
We accept checks or credit cards for payment.

This show is limited to 80 vendors. We accept
hand-crafted items made by you and your
family. Absolutely no resale items allowed.
By completing this application form, you are
acknowledging your products are handmade.
This is an OUTDOOR festival that takes place
rain or shine on grass in Ellison Park located
next to the Mississippi River. You must provide
your own display tables, chairs and/or canopy
shelter for shade and any other supplies
needed for the sale of your product. Canopy
tents are not required but most vendors bring
them. Please note that we are unable to
guarantee level display space. Electricity is not
available nor are generators allowed in the
park.
Exhibitors are responsible for reading and understanding the material presented in this application. Failure to abide by the rules/
regulations set forth by the Monticello Chamber of Commerce & Industry & the City of
Monticello may result in removal from current
and future shows.
Driving in the park is prohibited
Exhibitor is responsible for proper insurance during
the festival. The Art in the Park event and affiliated
organizations will not be responsible for damage to
work or property.

Applications postmarked by Friday, April 3, 2015
receive $10 off their booth space. All applications
due (postmarked) no later than July 1, 2015 for acceptance. All applications postmarked after July 1
will be subject to a $25 late fee. The last day to
cancel for the show is Fri. June 12, 2015 for a full refund. If you chose to cancel after this date there
will be no refund.

**Submitting this application form does not automatically
grant you entrance into the show. This completed application, ST19 form, description of your work (see back
panel for a reference), and photos must be included to be
considered for a space in the Monticello Art in the Park
festival. The event jury will review applications and decide accordingly which vendors will be accepted. The
decision of the jury is final.**
Include payment to reserve your booth space and be registered to participate in the show.
If you are accepted into the show we will send your
confirmation by e-mail no later than Wed. July 8,
2015 with the final details on the show. Questions
