2015-2016 Membership Contract
To be completed by the member:
I, ________________________________________, as a member of the Service League
of Boys, understand that membership in this organization requires attendance at a
minimum of 4 bimonthly meetings and fulfilling 10 hours of service in approved
philanthropies. I also understand that failure to fulfill these requirements may result in
termination of my membership.

To be completed by the parent:
I, ___________________________________________, as a parent of a Service League
of Boys member, understand that I have responsibilities in maintaining our membership
in the MVLA Service League of Boys. I am to attend a minimum of 2 bimonthly
meetings, and perform 5 hours of service in approved philanthropies. If I do not fulfill my
requirements, I understand I jeopardize my son’s membership in this organization. I also
understand that reporting of hours validates what we do as an organization, and I will file
hours with the hours coordinator in a timely fashion with my son.

Submitting your membership forms and payment
Two ways to submit membership forms and payment:
1) Submit by mail: Complete the forms and mail them along with a 4 x 6 photo of son and
parent(s) and your check for $75.00 per son (plus $10 for each additional t-shirt) payable to
MVLA Service League of Boys to: The Tongs, 435 Almond Avenue, Los Altos, 94022.
2) Submit electronically: Complete and save the forms and attach them to an email with the
subject "SLOBs Membership Registration". In the same email, attach a 4 x 6 photo of son and
parent(s). Send email to Esther Tong at [email protected]. Then, proceed to pay your $75
membership dues and additional t-shirt charges via Paypal (www.paypal.com). Below are the
instructions to send payment using Paypal:
1. Go to www.paypal.com. If you don’t have an account, set up a PayPal account first.
2. Click on “Pay or Send money" tab.
3. Select “Send money to family or friends" option.
4. Enter email "[email protected]”.
5. Enter amount you are sending.
6. Indicate member name and purpose of payment in message box.
7. Send money.
8. Forward a copy of your paypal receipt to [email protected].
