Print the ‘Summer LVN to ADN Application” form at the nah.southtexascollege.edu website. OFFICIAL STC transcript of records. Copy of HESI Admission Assessment (A2) scores within the last two years or ACT/SAT scores within the last five years. e. Copy of current LVN license. To expedite your application process, kindly comply with the following procedures: a. See Clinical Compliance Specialist, in NAH R-1.602 and get clearance with item #6 in the application form. “Students must complete and provide documentation to Clinical Affairs Specialist for clearance on their Background Check, Hepatitis B series or titer, and other requirements prior to applying to the LVN-RN Transition Track. Please see Clinical Compliance Specialist in NAH R-1.602 for clearance on all other requirements.” b. Mark a (√) in all the areas in the application form which apply to you. If you answered “NO” to questions #1-7, you must schedule an appointment with a faculty advisor before submitting your application. c. d. Highlight all pre-requisite and co-requisite courses to be credited on your transcript of records. Compute your GPA and total acceptance points by following the formula on page 2 of the application form. Complete and sign the application form.
