Copy of HESI Admission Assessment (A2) scores within the last two years or ACT/SAT scores

To expedite your application process, kindly comply with the following procedures:
a. See Clinical Compliance Specialist, in NAH R-1.602 and get clearance with item #6 in the
application form.
“Students must complete and provide documentation to Clinical Affairs Specialist for clearance
on their Background Check, Hepatitis B series or titer, and other requirements
prior to applying to the LVN-RN Transition Track. Please see Clinical Compliance Specialist in
NAH R-1.602 for clearance on all other requirements.”
