For the 2015-2016 school year, 5th grade Natomas Unified School District (NUSD) students may participate in the Middle School
Choice program. Please complete all sections of this form, including selecting the middle school in which your student is to attend for
6th grade during the 2015-2016 school year. The Middle School Choice form must be turned in by 4pm on March 20, 2015. This form
may be turned into any NUSD elementary school, middle school, or the Education Center. Students who do not return a completed
form will automatically be enrolled into the their middle school of residence. After the March 20th deadline, an intra-district transfer
request may be submitted for consideration.
