Precious Minds Support Services Introduction to BLAZE & CONNECT Summer Camp 2015! 31, 2015 March Dear Parent/Guardian, Thank you for your interest in Precious Minds Summer Camps! We are so excited to be planning another season of BLAZE Camp and CONNECT Camp for your child. 2015 is shaping up to be another great summer filled with fun outdoor activities, friendship building, crafts, music and more! Our camps run from 9-­‐4 each day at the Precious Minds Resource & Learning Centre, located at 21980 Highway 12, just north of Greenbank. CONNECT Camp is for kids (7-­‐13) and teens (high school age) with a diagnosis of high-­‐functioning autism, ADHD, mild intellectual disability, etc. and is an exciting summer camp opportunity to allow your child to meet new friends, learn and develop important social skills, play exciting cooperative games and express creativity through arts and crafts. At BLAZE Camp, we offer a fun, unique, safe, and individualized structured support program for kids (5-­‐13) and teens (high school age) with a developmental disability. This summer we will be running the following sessions of camp: Session 1 – Blaze (Kids & Teens) July 6 – July 17 Session 2 – Blaze (Kids) & Connect (Kids) July 20 – July 31 Session 3 –Connect (Kids) August 4 – August 14 Session 4 – Connect (Teens) & Blaze (Teens) August 17 – August 28 The cost for each session is $500 with the exception of S ession 3 ($450 -­‐ program is not running on Monday, August 3rd for the Civic Holiday). Steps for Camp Registration: ü Complete the 2015 Program Application and indicate your preferred choice of session(s) ü Complete the Client Profile Form. If you completed this form in the past 12 months, please just let us know if there are any changes. ü Read and sign the Program Policies & Participation Agreement. ü If you are a new family to Precious Minds, we will arrange a personal visit in order to gain a clearer understanding of your child’s needs. ü Prior to May 1, 2015, you will receive confirmation of what session(s) your child is registered in. An invoice will be provided at that time and payment will be due upon receipt o f the invoice. If you require financial aid, please complete our Financial Assistance Application and include it with your 2015 Program Application. Although our subsidy fund is limited and we cannot guarantee funding, we will do everything we can to ensure that financial barriers do not keep your child from participating at Precious Minds. We will also make every effort to assist you to secure additional funding from other sources. We look forward to the possibility of having your child with us this summer!
