FORMAT – Two Brackets – Boys 18 & Under – Boys 15 & Under

Qualifying will be 36 holes, 18 on Monday and 18 holes on Tuesday (July 27 & July 28 ). Match play will begin on

Wednesday, July 29 , with two matches on Wednesday, two matches on Thursday, July 30 , and a 36-hole final match

on Friday, July 31 . 32 players qualify in each boys’ bracket.
CADDIES

A contestant may provide his / her own caddy. Caddies must be under age 19 (18) as of July 31 , 2015.
ELIGIBILITY
st
Entries are open to amateur boys that under the age of 19 (18) as of July 31 , 2015. Contestants must be a legal
resident of Wisconsin. Proper golf attire is required. Walking shorts are permitted.
ENTRY FEE
Entry fee is $75.00 and must accompany entry blank. All entries are subject to approval by the Tournament Committee in
its discretion either before or during the Championship. Entry fee includes lunch on Monday and Tuesday at the Johnson
Park Club House, and Gifts for all competitors. Make checks payable to: Junior Masters.
DEADLINE
THE FIELD WILL BE CLOSED AT 200 PLAYERS OR JULY 13th, 2015, WHICHEVER OCCURS FIRST. LATE OR
INCOMPLETE ENTRIES ARE NOT ACCEPTABLE.

PARENTAL RELEASE AND INDEMNITY AGREEMENT
In consideration of our son, a minor, being permitted to participate, we, as parents and guardians of our minor son, for ourselves
and on behalf of said minor son, hereby release, waive and discharge The Mike Bencriscutto Junior Masters, its officers, directors, members,
promoters, employees and sponsors (releasees) from any and all liability to our son and ourselves for any and all injury, loss or damage
of any kind or nature, whether caused by the negligence of releasees or otherwise. We further agree to indemnify the releasees from any and all liability
for any injury, loss or damage of any kind to our minor son or ourselves, whether caused by releasee’s negligence or otherwise. We further
agree to indemnify releasee’s against any expense, cost and fees releasees may incur due to any action or claim brought by our minor son or
ourselves.
