$45.00 ($30 for 10-11 year-old Boys and 10-12 year-old Girls both of whom play nine holes)
registration fee includes 18 holes* of golf (walking, no carts) plus a sleeve of golf balls, hot
dog and pop, and trophies for top boy and girl qualifiers in each division. Note: junior golfers should play in a local qualifier to advance. Pre-registration only – no “walk-ons” permitted. Non-motorized pull carts allowed for lower divisions. See registration deadline and scoring rules on next page.

Date I hereby release the sponsors from any and all liabilities, which might occur during my child’s participation in this Optimist-sponsored Junior Golf Qualifying Tournament. I understand the top boy and girl qualifiers in each age group from
this tournament will advance to the Michigan District Qualifying Tournament (separate entry fee will be paid for 1st place
qualifiers) to be held June 23rd and 24th, 2015 at The Metamora Golf Course in Metamora, Michigan. Qualifiers advance
to the Optimist International Junior Golf Championships at the PGA National Resort and Spa in Palm Beach Gardens,
Florida in July.

QUALIFYING CRITERIA TO ADVANCE
*BOYS 16-18: a score of 82 or under
*GIRLS 15-18: a score of 90 or under
*BOYS 14-15: a score of 86 or under
*GIRLS 13-14: a score of 93 or under
*BOYS 12-13: a score of 93 or under
**GIRLS 10-12: a score of 48 or under (9 holes)
**BOYS 10-11: a score of 46 or under (9 holes)
**NOTE: 10-11 year-old Boys and 10-12 year-old Girls only play nine holes
