How to use the checklist
1. Before you sign your visa application form, use this checklist to make sure your application is complete.
2.
When you are satisfied that you have correctly completed your visa application form and all of the relevant
documentation is included, you should sign and date your visa application form.
3.
Include the completed visa application checklist when you lodge your visa application form and relevant
documentation at the Visa Application Centre. See: https://au.tlscontact.com/
Important information
Please be aware that the decision on your visa application may be based on the information and documentation that you
include at the time of lodgement. Applicants should not make arrangements to travel to Australia until advised that their
visa has been approved. You may be asked to provide information in addition to that provided in your application. If this is
the case, you will be given a date by which you will have to provide the information.
The provision of false or misleading information or documents may have an adverse effect on the outcome of the
application and will delay processing.
You must provide a certified copy of the documents, not the originals. Please be aware that any original documents you
choose to provide may not be returned to you. If an original is required, the Immigration processing office will request this
from you. All documents must be certified by the relevant authority. Documents not in English must be accompanied by
accredited English translations.
Delivery and courier charges
Applications can be lodged at the Visa Application Centre in Harare – Teleperformance with FedEX. To make an

You are responsible for all costs of delivering information to the Immigration processing office by mail or courier, including
any additional information that may be requested by this office. If you give additional information to the Australian Visa
Application Centre, you may be charged a courier fee for delivery to the Immigration processing office.

The Visa Application Charge will not be refunded if a decision is made to refuse to grant the visa because the applicant
did not satisfy the criteria for grant of the visa.

Complete this checklist and attach all documentation – in the order provided below. The checklist should be placed to the
front of your application.

A completed and signed Form 470F - Application for migration to Australia by other family members
Each person included in the form, who is aged 18 years or over, must have signed the Australian
values statement.


A completed and signed Form 40 - Sponsorship for migration to Australia
This form should be completed and signed by your sponsor.


A completed and signed Form 47A - Details of child or other dependent family member aged 18 years
or over.


A completed and signed Form 80 – Personal particulars for character assessment for each applicant
aged 16 years or older. Applicants must list full details of all addresses they have lived at for 12
months or more during the last 10 years and provide full details of all employment and / or education
since leaving school. If you believe a question is not relevant write “not applicable” and explain why.


Payment of the Australian Government Visa Application Charge at the Visa Application Centre or if the
Visa Application Charge has been paid at a DIBP office in Australia, a copy of the receipt must be
submitted.


Whichever is applicable, a completed and signed:
Form 956 - Advice by a migration agent/exempt person of providing immigration assistance or
Form 956A - Appointment or withdrawal of Authorised Recipient


Certified copies of the biographical pages of the current passports or travel documents of all people
included in the application (these are the pages with the holder's photo and personal details and the
issue/expiry dates).
Note: your passport must have at least six months left before the expiry date.


Two recent passport-sized photographs (45 mm x 35 mm), no more than six months old. You must
also include photographs of non-migrating dependent family members.


Documents to prove your identity – your birth registration showing both parents’ names.


If there are dependent applicants 18 years of age or over, evidence that they are financially dependent
on the main applicant for the basic needs of food, shelter and clothing, and how long this support has
been provided. Evidence may include bank statements, money transfers, rent receipts etc.

If you or anyone included in the application is, or has been married, the marriage certificate(s). If you
are living in a de facto relationship, evidence that you have been in the relationship for at least 12
months. For example: joint bank account statements, billing accounts in joint names etc.


If you or anyone included in the application has been permanently separated, divorced or widowed, the
statutory declaration/separation certificate, divorce decree absolute or the death certificate of the
deceased partner (as appropriate).


If an applicant has changed his or her name (for example, by marriage or deed poll), evidence of the
name change is required.


Penal certificate from countries where the applicant has resided for 12 months or more during the last
10 years (applicable to applicants 16 years old or over).

Two recent passport-size photographs of your sponsor (45 mm x 35 mm).
Evidence that the sponsor is an Australian citizen, an Australian permanent resident or eligible New
Zealand citizen.
Evidence that the sponsor is usually resident in Australia.
If the person requiring care is a member of the sponsor’s family unit, evidence of both of the following:

their family relationship with, or dependency on, the sponsor
their status as an Australian citizen, Australian permanent resident or eligible New Zealand
citizen.
The Medibank Health Solutions Carer Visa Assessment Certificate for the person who requires care or
evidence the Medibank Health Solutions assessment has started (a letter from Medibank Health
Solutions acknowledging your request for a care assessment).


Evidence to show that the sponsor has tried to access care or assistance in Australia from services
such as welfare, hospital, nursing or community services.
Evidence to show that the sponsor has no other relatives in Australia who can provide the required
care or assistance.


