Choose Vivity Premier HMO before June 1, 2015. 2 How to Enroll in the Vivity Premier HMO Step by Step Instructions 1. Go to the Benefits Portal at Cedars-Sinai.MyBenefitChoice.com and click on View Your Benefits to enter the Benefits Enrollment Site. 2. Because this is a new enrollment site, you’ll need to register for a private account to protect your personalized information, and select a new user name and password. (Passwords created before April 6, 2015 will not work.) The company key = CSHS. 3. Click Enroll Now, then choose a navigation option (Step By Step or Pick And Choose). 4. When you get to the Medical section, enter the dependents you wish to cover. 5. Under Select A Plan, select the plan you want to enroll in. 6. If you enroll in the Vivity HMO, you’ll be taken to the Primary Care Provider Selection page. Here, you will need to select a Primary Care Provider (PCP) for each covered family member. Look up the provider PCP Codes under Helpful Tips. (Call the MBC HR/Benefits Help Desk if you need help.) Fill in the provider code in the PCP Code box. Click Continue when you are done.
