The senior prom is an event planned and organized by students in our senior class and their class advisors. It is
sponsored by FWHS to help foster fun and class spirit in a safe and wholesome environment. Students and their
guests who attend this function are expected to abide by all school rules and policies that govern student behavior.
Use of alcohol or any other behavior altering substance before, during, or in any way associated with this
function is strictly prohibited.
Guests of FWHS students are welcome at this event. Each guest is expected to sign this agreement and abide by all
rules and policies of Fairfield Warde High School. Failure to do so may result in disciplinary consequences for the
FWHS host, legal action when appropriate, and loss of any opportunity to attend an FWHS event in the future.
Fairfield Board of Education policy requires that all students attending the dance take a breathalyzer
test. Any student who fails the test will be denied admittance and will be subject to the consequences
outlined in the alcohol and substance abuse policy. Students may also be breathalyzed upon exiting the
dance.
Signatures on this form indicate students’ and parents’ understanding of this agreement and constitute a promise to
abide by all school rules and regulations.
