ENROLLMENT INFORMATION FOR 2015-16
Purple colored enrollment forms for the 2015-16 school year have been sent home with all K-5
students who are currently enrolled in Highland Terrace. Please take a moment to complete this
form so that Highland Terrace Elementary will be aware if your child is planning on returning to
our school in the fall or not. Every Highland Terrace student should return a completed
“purple sheet” to their teacher. Thank you.
Hello Highland Terrace Families-Please help us pass the word!! Students and staff at HT are experiencing a record number of
illnesses including stomach viruses (with all the joys that come with that!) and eye infections. In
addition there have been a high number of respiratory illnesses.
We need our students to be washing hands, with warm water and soap FREQUENTLY! Washing
hands is the number one way to prevent the spread of illnesses. Hands should be washed every
time the bathroom is used, every time hands get dirty (like after recess), and before eating. Hand
sanitizer is fine before snacks and lunch but not after bathroom breaks or if hands are visibly
soiled. Teachers will be emphasizing this, but we need families to be doing the same.
Another way to prevent the spread of germs is to sneeze or cough into an elbow. Please review
this with your child, and adults--remember to be a good example! We want to keep all those
droplets that come from the mouth and nose out of the air as much as possible!
Please remember that students who have vomited or had a fever of over 100 within the past 24
hours should NOT be coming to school. Please follow this guideline when deciding whether or not
to send your child to school. If you have any questions please call the nurses office. We are happy
to talk to parents at any time.

Sports Physicals for middle school 2015-16
The whole subject of sports physicals can be very confusing—when you need them, how long they
last and even what form to take to your doctor. As students enter into 7th and 9th grades in the
Shoreline School District, sports physicals (officially known as “Secondary Student Health Report”)
are required to be able to try out and play in a sport after school. It is recommended that a doctor
appointment be scheduled sometime after June 1ST of the current year. The sports physical is good
for 24 months based on the date of the actual physical exam by the health care practitioner. The
left side of the form is completed by the parent/guardian and the right side is completed by your
health care provider. The entire sports packet includes forms for Code of Conduct, Physical
Examination, Concussion Information, Athlete Information, Permission, and Insurance (please
make sure to complete the ENTIRE page), Middle School Athletic Training, Form packets can be
picked up at anytime in the Einstein Middle School office. These forms can also be downloaded
from the Einstein web page under Athletics, Registration tab. Fall sports usually start the first
week of school so it is imperative that these forms be turned in that week if your student wishes

TIME FOR SMARTER BALANCE TESTING
Just a friendly reminder for days scheduled for testing. Please make sure they are ready for the
day with good sleep the night before, a good breakfast, and no early dismissals. Take a practice
test and see for yourself. Here’s the link: http://www.smarterbalanced.org/pilot-test. Click on black
line titled “Smarter Balance Assessments”. Then “Practice Tests and Training Tests”, then “Take
the Practice and Training Tests” (in green). On the bottom in a green box, click on “Student
Interface Practice and Training Tests”. Click “Sign In”, select your student “Grade”, and click “Yes”.
Select one of the tests. This is an example of what you child will be tested on.

exam, fluoride application, teeth cleaning, sealants, and more. Join us for a day of service, educational activities, and
