Applicant must be between 11 to 18 years old and a resident of Quebec. Applicant must have a significant need for orthodontic treatment with fixed appliances (braces). Applicant must be a student currently enrolled at full time. Applicant must demonstrate financial need and submit a proof of income. Copies of the two following documents pertaining to the most recent Federal personal tax return of the applicant’s parents or guardians are required: the 4 pages of the Federal personal tax return and the related notice of assessment. Applicant must agree to follow the treatment plan elaborated by the orthodontist, as well as demonstrate the ability and commitment to make all appointments on time. Applicant is encouraged to display involvement and leadership in extracurricular activities, and to demonstrate an overall positive attitude. Applicant must be willing during the treatment period to “pay it forward” by completing 20 hours of community service each year. Two letters of recommendation are mandatory. Please do not submit more than two letters, and limit each letter to one page. Please make sure that they are typed or printed clearly with black ink (no pencil). Letters of recommendation may be written by anyone except by family members: teacher, coach, counselor, dentist, etc. The person writing the letter must include his or her phone number so that he or she can be reached if necessary. Two clear 5x7 headshot photographs must be included with the application: one showing the whole face with full smile and one close-­‐up shot of the smile itself showing teeth. The application, including the letters of recommendation and the photos, will not be returned and will become property of Smile for a Lifetime Foundation. Applicant must agree, he or she is accepted, to having pictures of himself/herself taken during the treatment period with the treating orthodontist to be used by Smile for a Lifetime Foundation. Applications are received on an ongoing basis, but will only be reviewed once a year. Each applicant will be notified of approval or denial after the end of each selection process. Return the completed application consisting of the following documents: application form, applicant questionnaire (handwritten and answered by the applicant), general dentist form, two letters of recommendation, two photographs (face and smile), copy of the applicant’s most recent report card, copy of the 4 pages of the most recent Federal personal tax return of the applicant’s parents or guardians and copy of the related Federal notice of assessment, together in one packet to the address below. Please note that applications that do not meet these criteria will be considered incomplete and will not be voted on by our Board of Directors.
