The Phoenix Medical Practice
The Phoenix Medical Practice is a small, friendly, well
established, single-handed, undergraduate teaching practice.
Dr Khan is the principle GP, and your named accountable GP. The
Practice is proud to offer the highest standard of patient centred
healthcare. We run many clinics for the management of chronic
disease (eg, asthma, CHD, diabetes) and offer a wide variety of
other medical services including minor surgery, childhood
vaccinations and well person checks.
We are a member of the Family Doctor Association (FDA). The
Family Doctor Association leads the way to the future for grassroots
primary care. With over 1000 members inspired for over 30 years.
Continuity of care benefits patients, improves health outcomes and
saves the NHS money. The member GP practices aim to provide
high quality medical care while maintaining the traditional personal
care of the family doctor in a friendly environment promoting health
by education and preventative measures. For further information

Disabled Access
To enable access to the surgery for disabled patients we have the
facility of a ramp and double doors for easy access. The practice is
based on the ground floor level only.

Patient Group (PPG)
The practice has a successful virtual patient representation group so
that you can have your say. We will ask members of this
representative group some questions from time to time, such as what
you think about our opening times or the quality of the care or
service you received. We will contact you via email and keep our
surveys succinct so it shouldn’t take too much of your time. If you
are happy for us to contact you occasionally by email, please ask the
receptionist for a sign up form or go onto our website and complete
the sign up form. We will be in touch after we receive your form.
Please note that no medical information or questions will be
responded to. We welcome all patients of the practice to join.

A range of nurse and doctor morning and afternoon appointments
are available. If you wish to see a particular practitioner, please
inform the nurse/receptionist when booking an appointment.

T.A.R.G.E.T. Staff Training
The practice is closed on some Wednesday afternoons for
T.A.R.G.E.T. Training from 12.00pm. This enables all staff
(doctors, nurses and administration) to keep fully up to date with
modern medical practice in Primary Healthcare.
When We Are Closed
If you need urgent medical advice or assistance when the practice is
closed, please phone the practice on 01302 323992 and you will be
transferred to the Out of Hours Service.
The Out of Hours Receptionist will take your details and a duty GP
will phone you back to discuss the problem in further detail. The
practice receives details of this contact the following day; this
enables us to update our patient medical records.
Call 999 in an emergency. Chest pains and/or shortness
of breath constitutes an emergency.
Appointments
Making an Appointment & Nurse Triage
Appointments can be made by phoning the receptionist on 01302
323992. When you phone for an appointment the receptionist will
need to take a few details about your problem, so the nurse can
phone you back, this ensures that your problem is dealt with most
appropriately. This is called Nurse Triage.
You can inform the receptionist or triage nurse if you have a
preference of practitioner.
Please remember that an appointment is only for one person.

Patients who have not been reviewed for over three years can
request a review appointment and consultations are available for
patients aged over 75.
Patients Who Do Not Attend (DNA) for Appointments
We are a busy practice and when patients DNA, valuable doctor and
nurse time is wasted resulting in less available appointments for
those patients who need them. We have a policy for patients who
DNA without giving prior notice, which may result in their removal
from our practice list.
If you cannot keep your appointment, please cancel as soon as
possible.
SMS Text Messaging Service
If you have a mobile telephone number, please inform the
receptionist of this number so that we can update your records. We
currently use an SMS messaging service that will send you
reminders of appointments at the surgery. From 1 April 2016 this
will happen automatically, so you need do nothing unless you do
not wish to receive these reminders. If you do not want to receive
them please see the receptionist or complete a dissent form to opt
out of this service.
Home Visits
Home Visits are reserved for those patients who, for medical
reasons only, are unable to attend the practice.
Home Visits should be requested before 10.30am where possible.
Visits are at the discretion of the doctor. The receptionist will need
to take a few details about your illness in order to pass onto the

nurse or the duty doctor who will then ring you back to gain more
information about your problem and deal with it appropriately.
Telephone Advice
If you do not require an appointment but would like to speak to a
Clinician, please telephone the surgery on the Practice number and
the Receptionist will take the message which will be passed to the
Clinician for action. Please note that any urgent medical matters
should go through the nurse triage system.
Medical Students
We are an undergraduate teaching medical practice and as a result
have attached medical students from Sheffield University School of
Medicine who may be with Dr. Khan at the time of your
appointment. If you object to this then please inform the reception
staff.
Hospital Referrals
If you need a referral to the hospital we use an online booking
system (e-referral) which allows you to choose your hospital or
clinic and book an appointment with a specialist at a time to suit
you.
CQC (Care Quality Commission)
The Phoenix Medical Practice is registered with the CQC. The
CQC makes sure that hospitals, care homes, dental and GP
surgeries, and all other care services in England provide people with
safe, effective, compassionate and high quality care, and encourages

them to make improvements. We do this by inspecting services and
publishing results on our website to help you make better decisions
about the care you receive. For more information, visit

Confidentiality
All our staff have signed a Confidentiality Clause as part of their
employment contract. This means we will not disclose information
to your family, friends or colleagues about any medical matter at all,
unless we know we have your written consent to do so.
The Protection & Use of Patient Information
There may be times when we may be required to share some of your
information with other NHS staff, e.g. hospital doctors, nurses etc.
This is necessary to ensure you receive the best possible health care
and treatment. The law says we must pass on certain information,
for example, to notify a birth. The NHS Central Register for
England and Wales contains basic personal details of all patients
registered with a General Practitioner (GP). The register does not
contain clinical details.
Everyone working for the NHS has a legal duty to keep information
about you confidential.
We are registered and comply with the Data Protection Act 1998.
Care Data
Important information about you and the care you receive is shared,
in a secure system, by healthcare staff to support your treatment and
care. It is important that we, The NHS, can use this information to
plan and improve services for all patients. The NHS would like to
link information from all the different places where you receive
care, such as your GP, hospital and community service, to help them

provide a full picture. This will allow the NHS to compare the care
you received in one area against the care you received in another, so
it can see what has worked best.
Information such as your postcode and NHS number, but not your
name, will be used to link your records in a secure system, so your
identity is protected. Information which does not reveal your
identity can then be used by others, such as researchers and those
planning health services, to make sure we provide the best care
possible for everyone.
You have a choice. If you are happy for your information to be
used in this way you do not have to do anything. If you have any
concerns or wish to prevent this from happening, please speak to
practice reception staff.
Freedom of Information
Information about the practice required for disclosure under this act,
should be made to the practice manager. (this does not include
access to patients medical information)
Access to Records
In accordance with the Data Protection Act 1998 and Access to
Health Records Act, patients may request to see their medical
records. Such request should be made through the practice manager
and may be subject to an administration charge. No information will
be released without patient consent, unless we are legally obliged to
do so.

Comments & Complaints
We are very keen to receive your comments about the service that
the practice provides to its patients. If you would like to comment
on any particular aspect of our service, or if, unfortunately, you have
cause to complain, please contact Mrs Paula White, Practice
Manager on the practice number.
The Health Service Ombudsman
The Health Service Ombudsman has published a booklet that
describes the ‘six principles for remedy’ in relation to complaints

Patients who are Violent and Aggressive
The Practice will not tolerate violent or abusive behaviour to staff or
persons on the practice premises. Such actions will result in
immediate removal from the premises and practice list. The practice

will notify Patient Services immediately of any removal of a patient
from the list.
Ordering of Prescriptions
We require 48 hours to process a repeat prescription
You can order prescriptions by:

Post or fax - please remember to tick the medicine you need
on the medication list supplied with your previous repeat
prescription
Telephone – telephone requests for repeat prescriptions are
taken after 11am only
Online using the Doncaster PCT Repeat Prescription Service
In person – we have a deposit box for repeat prescription
requests in the entrance to the surgery
Please allow 48 hours before collection
Ordering Prescriptions Online (SystmOnline)
If you have access to the internet, it is now possible to order your
repeat prescription free of charge from the practice on-line. This
service will be available 24 hours a day, 365 days each year apart
from short periods when the website will be having essential
maintenance carried out. This website has been fully tested and has
been accredited as being secure. The availability of this service is
governed by the practice and is dependent upon responsible use by
you, the patient. If not used responsibly, access to the online
services will be revoked by the practice.
Please ask at reception to be set up with a login/password.

SystemOnline App
This is now available to patients as an app for iPhones, iPads and
other Apple devices running the iOS operating system. The app is
listed on the App Store and is free to download. The app is also
available for android devices.
To download the app, search for “SystmOnline” in the app store.
Patients can use their existing SystemOnline login details to access
the online services offered by the practice via the app. The app
provides a quick and easy way to change your contact details, order
repeat prescriptions and view appointments. Please note that the
App provides only basic functions for SystmOnline and you may
need to log on via your PC for more advanced tasks.

Community Midwifes
Please contact the Midwife via the Midwife Triage System on
01302 647071 (between 9am and 7pm) if you need to speak to a
midwife.
Community Nurses
Our district nurses visit people at home when they are unable to
come to the practice for treatment. They give help, advice and
treatment for a wide range of conditions. Contact them directly on
01302 566999.
Health Visitors
The health visitors attached to our surgery take a special interest in
the under 5’s and over 60’s. they advise on general welfare matters
and availability of help etc. They can be contacted directly on

Clinics
The practice offers the following clinics/services:-
Healthy Eating and Weight Management
If you require any help and advice on healthy eating or weight
management please contact the surgery to speak to a Nurse. We can
offer regular appointments to help you with your weight
management goals
Flu Clinic
We offer annual flu vaccinations for patients in ‘at risk’ groups. If
you would like a flu vaccination and fall into one of the following
categories, please contact the surgery between October and
December for an appointment









Over 65
Chronic Respiratory Disease including Asthma
Diabetes Mellitus
Chronic heart Disease
Chronic Renal Failure
Immunosuppression due to disease or treatment
Chronic Liver Disease
Pregnant
Carer for an elderly or disabled person whose welfare may
be at risk if you fall ill

Respiratory Clinic
Your GP will refer you to our respiratory nurse if they feel it
necessary. If you have asthma or any similar respiratory condition
our nurse will wish to see you for yearly reviews. You will be sent
an appointment letter when you are due for a review. The
Respiratory Clinic is usually held on a Wednesday morning.
Diabetic Clinic
If you are diabetic our diabetic nurse will wish to see you either
yearly or 3 or 6 monthly for reviews which include blood tests and
medication reviews. You will be sent an appointment letter when
you are due for a review. The Diabetic Clinic is usually held on a
Monday morning.

Healthy Heart Clinic
We currently hold annual review clinics for patients with heart
disease. All patients with diabetes who also have heart disease will
have this check-up incorporated into their diabetic checks. You will
be sent an appointment when you are due for a review, this will be a
30 minute appointment with the practice nurse, which will involve
having a fasting blood test, a blood pressure check and you will be
asked questions about your general health. The Healthy Heart Clinic
is usually held on a Wednesday morning.

Baby Vaccination and Child Surveillance Clinic
We hold a weekly clinic every Tuesday morning for routine
baby/childhood vaccinations. You will be invited to attend the
practice when your child's immunisations are due. Please note that
this is NOT a drop-in clinic and an appointment is necessary. You
will be sent an appointment directly from the Child Health
Department. When you attend for appointments please remember to
bring your child’s red book with you.
The GP carries out a child health check on babies at approximately
8 weeks of age. This is usually done at the same time as the first
immunisations.

Vaccinations
Routine Vaccinations for all ages are available during surgery times
by appointment with our practice nurses.

Cryotherapy/Wart Clinic
We run a Cryotherapy Clinic every few weeks. Please note that this
service is only available to book following referral by the doctor or
practice nurse.
Ear Syringing
All our nurses are trained on ear syringing. If you would like your
ears checked for this please contact the practice and speak to one
our our triage nurses who will make you an appointment. Please
note that for successful ear syringing it is recommended that you
soften the ear wax with olive oil for a week prior to your
appointment - the nurse will advise you of this when you ring for an
appointment.

ECG's
This is a tracing of the rhythm of your heart and can be carried out
by a practice nurse. You will need referral from a GP or nurse
before this can be done.
Family Planning
Advice can be sought from both the doctors and nurses. We provide
the following services:

If you require a coil fitting you can arrange to have this done at East
Laith Gate House telephone 01302 640040 for an appointment.

Minor Surgery
The practice offers regular minor surgery clinics covering
procedures such as removal of sebaceous cysts and suspicious
moles. You will need an appointment with the GP for an assessment
prior to being referred to this clinic.
Blood Pressure
It is recommended when you are over 40 years of age and over that
you have your blood pressure checked at least every 5 years. Please
contact the surgery if you wish to make an appointment.
If you have Hypertension you will be sent appointments when your
blood pressure is due for checking.

Blood Tests
If your doctor recommends you have a blood test you can either
make an appointment here at the practice or if it is more convenient
for you, you can attend the hospital. If you want to go to the hospital
for your blood test you will need a blood form from the practice to
take with you.
We are dependent on the hospital collection service for transporting
the samples to the laboratory for the testing, therefore blood test
appointments are only available between 8.00am and 2.00pm
Monday to Friday. There are certain exemptions to this rule for
certain blood tests. Please speak to the practice nurse if you need
further information regarding this.
The hospital drop-in phlebotomy services are as follows:
Doncaster Royal Infirmary Monday - Friday 8.00am - 5.00pm
Bassetlaw Hospital Monday - Friday 8.00am - 5.00pm
Mexborough Montagu Hospital Monday - Friday 8.00am - 5.00pm

Cervical Smears
The health authority sends reminders to patients when they are due
to have a cervical smear test. Female patients aged between 25 and
49 are called every 3 years and patients aged between 50 and 65
every 5 years. These tests are performed by our female practice
nurses. A 20 minute appointment is required and you should be midcycle at the time of testing. You can bring a chaperone with you to
the appointment if you wish. Please ring the practice to arrange an
appointment.
You can also arrange to have your smear done at the Sexual Health
Clinic at East Laith Gate House telephone01302 640040 for further
information.
Smoking Cessation
If you are a smoker and are interested in stopping smoking you can
telephone Yorkshire Smokefree on 0800 612 0011 for further
information on what services they can offer.
Health Visitors and Child Health Checks
Child Health Clinics are run by the health visiting team. Please
contact them on 01302 645557 for further information.
