Type 2 Medical Practitioners – Self
Assessment of Tiered Contributions
NHS Pension Scheme employee tiered contributions for GP Scheme members are based on
their total GP pensionable pay in the relevant ‘pensions year’.
The self-assessment form was introduced 2009-10
Every type 2 medical Practitioner in England and Wales is legally required to complete a selfassessment form on an annual basis.
The self-assessment form is available on NHS Pensions website by clicking onto ‘Member
Forms’ or alternatively at: http://www.nhsbsa.nhs.uk/2529.aspx
A type 2 medical Practitioner is:
• A salaried GP formally employed by a Practice or by a PCT/LHB.
• A long-term fee based GP who works for a Practice or a PCT/LHB.
• A salaried GP (or a long-term fee based GP) who works for an APMS Contractor (or a
sPMS Contractor) that is a Scheme Employing Authority (EA).
• A GP who solely performs OOHs either on an employed or self-employed basis.
• A GP ‘with special interests’ (GPwSI) employed or engaged by a PCT /LHB.
The pensionable pay figure column on the form refers to the actual pay on which you paid
pension. It is preferable when you show practice income, that the figures have been obtained
from a P60, or some other annual summary from the practice, as any adjustments on practice
income will be made using these figures, via the practice.
Part –time work should not be adjusted to whole time equivalent when completing the form.
