ADDENDUM #1
RFP #3001DB Certified Behavioral Analyst Services
This addendum serves to provide the following qualification change and to provide answer to
questions received for RFP #3001DB Certified Behavioral Analyst Services. This addendum
along with Addendum 1 Questions and Answers are posted on our website as a Word Form and
a PDF file. To download this document, go to http://lake.k12.fl.us, click on Departments, click
on Purchasing and, click on Bid Opportunities and scroll down the table to the Certified
Behavioral Analyst Services and click. The question period for this RFP has expired. No
further questions will be answered.
Delete the strike through portion of following paragraph found on Page 9, Submittal B:
Qualifications
Respondents must specify experience in providing Certified Behavioral Analyst Services. Board
Certified Behavior Analysts performing Behavior Analysis services for the Lake County Schools
must be duly qualified and certified as a Board Certified Behavior Analyst (BCBA) or a Florida
Certified Behavior Analyst (CBA) with full privileges to practice Behavior Analysis in accordance
with the laws of the State of Florida.
Please note that the opening date has not been changed. All RFPs are due in the Purchasing
Department, 29529 County Road 561, Tavares, Florida 32778 by no later than 2:00 P.M. on
Thursday, April 2, 2009.
Acknowledgement of Addendum by Vendor:
This addendum shall be completed by Vendor and returned with the RFP Package. If a RFP
package has already been submitted, this addendum must be submitted to the above address
in a sealed envelope, which is marked on the outside: Addendum to RFP #3001DB Certified
Behavioral Analyst Services. This is to acknowledge receipt of this addendum, which will
become a part of the proposal document.
This addendum must be signed and returned in order for your RFP to be considered.

ADDENDUM #1 QUESTIONS AND ANSWERS
RFP #3001DB Certified Behavioral Analyst Services
1.
What was the exact unit cost per hour for Lot 1 and Lot 2? $85.54 per hour
was the unit cost for both Lot 1 and Lot 2.
2.
What was the total cost of lot 1 and Lot 2 for each previous contract year?
The total cost for Lots 1 and 2 for 06-07 was $124,204.46 & the total cost Lots
1 and 2 for 07-08 was $286,337.75.
3.
Was The Lake County School Board satisfied with the services of the last
vendor? Yes
4.
How many companies participated in the last RFP for these services? Three
(3)
5.
The RFP states that each BCBA shall be trained and hold current certification
in the Non-Violent Crisis Institute’s Crisis Prevention Intervention (CPI) at no
cost to the district. Will the district accept certification in Professional Crises
Management (PCM)? PCM is the only program currently approved through
the Agency for Persons with Disabilities. No, if not trained in CPI, we will train
them as soon as they begin but the company may not charge us for the time
they are involved in training or for the cost.
6.
Who is considered a Florida School Board customer? This is referring to other
School Boards in the State of Florida for whom you may have performed
services.
7.
Will Lake County School Board consider a licensed school psychologist for
this position? Not unless that person also is a Board Certified Behavior
Analyst with school experience preferred.
8.
Who is the current contractor(s) for both Lots 1 and 2? Agency for Behavioral
Services is the current contractor for both Lots.
9.
What is the current hourly rate under these existing contracts? See answer to
question 1.

10.
Is time spent outside the classroom by the BCBA in the preparation of
assessment reports and related tasks billable? Only if prior approval from
ESE Administrator is obtained.
11.
How will the schedule of visits and places be coordinated? Provider will
schedule according to weekly allocations per classroom per school. District
will approve proposed schedule.
12.
If travel to a site or school is given to the contractor and child is not available,
will services be paid? Travel time is not reimbursed.
13.
Are the hours of planning able to be provided off site from the school? Only if
prior approval from ESE Administrator is obtained. Primary contract hours
are to be spent in specific classrooms.
14.
Is the school district satisfied with the current services provided and meeting
the school district needs? Yes.
