Open Enrollment through Benefitfocus for all Benefit Eligible Employees
Open enrollment for the 2016-2017 school year for employee benefits will be
June 1, 2016 – June 30, 2016. All changes for the 2016-2017 school year must be made at this time.
No changes to health insurance benefits will be allowed after the open enrollment period ends on June
30, 2016.


If you are adding a dependent to your health plan, you must provide the following
information:
 For a Spouse - marriage certificate and first page of your 1040 tax form.
 For a Child - copy of their birth certificate.
If you do not need to make any changes to your dependents then no action is required.
Please note, according to IRS regulations, you must enroll in Flexible Spending yearly.
IRS also relaxed the “Use It or Lose It Rule” in October 2013, allowing you to roll over up to
$500 to the next plan year without losing it. (FOR MEDICAL FLEX SPENDING)
In order to make changes, you can access the website through hr.qacps.org, click on “Benefits” then
“BenefitFocus.” You will need your user ID number which is your first name, last initial and last 4
digits of your SS #. Your temporary password will be your social security number without dashes. The
system will prompt you to change your password at that time. Also, the “Benefit Overview” can be
viewed on hr.qacps.org by clicking on “Benefits.”
Enclosed are the proposed rates for the health insurance deductions effective September 1, 2016.
Please be aware these rates are subject to change due to ongoing negotiations and funding.

IMPORTANT INFORMATION-BENEFITFOCUS OPEN ENROLLMENT
Open enrollment for the 2016-2017 school year will be June 1st through June 30, 2016. Any changes would
include deletion of dependents, the addition of eligible employees, or dependents not currently covered. These changes
will be effective September 1, 2016 through August 31, 2017.
Please note that additions to coverage are only allowed during the open enrollment period, except for the addition
of a newborn child within 30 days of birth or to add a spouse within 30 days of marriage.
Your online benefits open enrollment period through BenefitFocus will be June 1 – June 30, 2016.
Please be aware that you will be unable to make any changes to your benefits after June 30, 2016.
The effective date for open enrollment changes will be September 1, 2016.
