NHS
Intermediate care teams provide a short term rehabilitation service following illness
or accident to adults, generally aged over 60, but with younger adults considered on
an individual basis.
The service is provided in the person’s own home wherever possible. Where this is
not possible the service has intermediate care beds in local residential and nursing
homes.
The service promotes faster recovery from illness, avoids inappropriate hospital
admission, supports earlier discharge from hospital, avoids the need for long-term
care and maintains independence.
Intermediate care works closely with Joint Care Management Teams to ensure that
people are assessed for longer term services where appropriate.
