The newsletter is to provide patients with any news, information or forthcoming events. If you have any suggestions as to what you would like to see in your newsletter please inform a member of staff. Care.Data You may or may not have received through your letter box a leaflet called ‘Better information means better care’ notifying you of sharing information from your health records to the Health and Social Care Information Centre (HSCIC). The upload of patient information was originally scheduled for Spring 2014 and has now been delayed until Autumn 2014 as this will give more time for patients to decide and act on whether or not they consent to their information being shared. We have an opt-out form at the surgery for patient’s to complete if they do not consent to their medical information being shared to the HSCIC. If you do consent then you do not need to do anything further. If you have opted out of the Summary Care Record you will still need to complete an opt-out form for Care.Data as the two are totally different. You can get more information and also download a copy of the leaflet ‘Better information means better care’ at the NHS Choices website at www.nhs.uk/caredata You can also call a dedicated patient information line on 0300 456 3531. More details about how the NHS look after confidential information and how it may be used can be found on the website at www.hscic.gov.uk/patientconf 12/08/2017 1 Issue 15 Changes to Appointment System We have recently changed our appointment system so we have more appointments available to book within a 7 day period, most patients’ need to see a GP within a few days so hopefully this will provide more choice of days and times. You will not be able to book an appointment beyond 7 days, hopefully this will reduce the amount of patients who forget to attend their booked appointment. If the GP wants to see you again then the GP will ask you to book your next appointment at reception on your way out. Nurse’s appointments are still available to book up to 12 weeks in advance. Bank Holiday Dates Please make a note of the dates below of our opening times over the Easter and May bank holidays. If you need a doctor in an emergency please telephone NHS 111. Friday 18th April Closed Saturday 19th April Closed Sunday 20th April Closed Monday 21st April Closed Tuesday 10th April Open as normal Monday 5th May Closed May Day Monday 26th May Closed Spring Bank Holiday Good Friday Easter Monday Holiday Vaccinations Please allow 6 weeks for holiday vaccinations, if you need to make an appointment please book with the Practice Nurse and ask for a travel appointment, you will be asked to collect and complete a travel questionnaire and bring back to the Surgery prior to your appointment, this will give the Nurse vital information on where you are visiting and which vaccinations you require. Changes of Address and Telephone Numbers Please let the receptionist know of any changes to your address as soon as possible. Every patient must call into the surgery in person with a completed change of address form obtainable from reception and bring 2 forms of identity, for a list of acceptable forms of identity please ask at reception. If you are unable to get into surgery eg you are housebound then please telephone reception for further information. Please let us know of any telephone numbers you can be contacted on, e.g. Home, Mobile or Work as this will provide a more efficient service if we need to contact you. If your home telephone number does not accept incoming calls please provide us with an alternative contact number. 12/08/2017 2 Issue 15 NHS Health Check If you are between the ages of 40 and 74 you are eligible for a free NHS Health Check. An NHS health check is where your risk of heart disease, kidney disease, stroke and diabetes will be assessed through some straight forward tests and simple questions about your lifestyle and family medical history. If you would like to make an appointment for this, then please contact reception and book with the Practice Nurse and state you would like an NHS Health Check. Feedback Cards Please complete a comment feedback card located in reception and put in the box provided. Your comments will be noted and the information will help us to provide a better environment and service to you. Spring into summer Clocks go forward by 1 hour on Sunday 30th March 2014. Staff Training Days Please make a note of the dates below when the surgery will close. Wednesday 19th March Wednesday 9th April Wednesday 21st May We will close at 12noon on the day of training and re-open the following day at 8.00am. If you need a Doctor in an emergency please telephone NHS111. SystmOnline – Appointments Why not book your appointment online via our website. Ask at reception for your unique ID and password. Once you have these, go to www.warrengate.org.uk and follow the online appointments link on the right hand side. Missed Appointments During the months of December, January and February a total of 599 patients failed to attend their appointment this equates to 6642 minutes of wasted time. Please telephone the surgery if you are unable to attend your appointment then the appointment can be used for someone else who needs it. 12/08/2017 3 Issue 15 Telephone Consultations If your condition does not require a face to face appointment, we do have the option of a telephone consultation with a GP. If you require this service your name and a contact telephone number is recorded and the GP will call you back after 11.30am. Staff We recently said goodbye to Marie who has taken retirement after 26 years of service. Many of you may know Marie as our previous Practice Manager we wish her a very healthy and happy retirement. We would like to welcome Luke who is our new apprentice. You will meet Luke on our reception desk and prescription desk. Julie our office manager is also now a trained phlebotomist, she is running 1 or 2 blood clinics every day. Finally we would like to congratulate Andrea who now has the position of full time secretary. Unacceptable Behaviour We value and care for our staff. We would ask that they are treated with courtesy and respect. Any individual not complying with this request will be asked to leave the premises and may be removed from the Practice List. Summer Newsletter The Summer Newsletter will be issued in June 2014.
