JOB DESCRIPTION CONSULTANT OPHTHALMOLOGIST WITH AN INTEREST IN GLAUCOMA Introduction This is a 10 PA post which has arisen due a relocation. We are looking for a dynamic, forward thinking general Ophthalmologist with an interest in Glaucoma to join our team of 8 consultants. The successful applicant will be expected to contribute to service redesign initiatives. 1. GENERAL TRUST INFORMATION South Devon Healthcare Foundation Trust (SDHFT) manages Torbay Hospital, a 450 bed acute hospital with a catchment area of 300 square miles. The hospital provides an extensive range of secondary (acute) services serving a resident population approaching 300,000 people. This patient flow swells by as many as 100,000 visitors during the summer holiday season. Approximately 140,000 people live within Torbay (Torquay, Paignton and Brixham), 115,000 within Teignbridge and 25,000 in the South Hams area of South Devon. From south Dartmoor the area stretches to the length of the coastline covering Dawlish, at the mouth of the River Exe, past the Teign and Dart estuaries and up to Dartmouth. South Devon Healthcare was established in 1991 and became one of the first NHS foundation trusts in March 2007. South Devon has a significantly older population (+65) and this demographic, which is ahead of some other parts of the country by as much as 20 years, means that healthcare need is expected to increase by 15-20 per cent over the next decade. This increased demand will have to be met within existing resources. To achieve this, the Trust has established a vision for a fully integrated health and social care community centred on a motivated and flexible workforce that is innovative in its approach for continuous improvement. Performance and achievements The Trust meets all the five standards as set out by the Care Quality Commission (CQC) which include: treating patients with respect and involving them in their care, standards of patient care and standards of staffing. The Trust consistently meets, and often exceeds, a range of performance indicators for: Eighteen week referrals — with many specialities 11 weeks Outpatients and day case surgery Four hour waits in A&E Ambulance/patient handovers Cancer diagnostics and treatment Stroke care Diabetes Infection control Finance. Governance Clinical services at the acute Trust are delivered on a Directorate basis, there are 14 Directorates each with a Clinical Director. Clinical Directorates sit within three divisions (Medical Services, Surgical Services and Women’s, Children’s, Diagnostics & Therapies Services). Each division has an Associate Director of Nursing, an Assistant Medical Director and a Divisional General Manager. Commissioning The services provided by the hospital are funded by our NHS commissioners, the main being South Devon and Torbay Clinical Commissioning Group. The CCG receives an allocation of money from the government annually and money is provided to the hospital matched against priorities set by both the CCG and the government body called NHS England. Local health service configuration South Devon Healthcare NHS Foundation Trust manages Torbay Hospital. The area’s nine community hospitals have approximately 180 inpatient beds and are managed by Torbay and South Devon NHS Care Trust. The locality’s mental health services were transferred in November 2001 to the county’s specialist mental health trust, the Devon Partnership Trust. 2. TORBAY HOSPITAL Torbay Hospital provides acute services to South Devon with a 10-bedded Critical Care Unit, 6 general theatres, 3 orthopaedic theatres and a day surgery unit. Services in all specialties are provided within the hospital with the following exceptions: Renal Medicine and Plastic Surgery services are provided by the Royal Devon and Exeter Hospital, Exeter, and at Plymouth Hospitals NHS Trust, with outpatient and day case sessions held at Torbay; satellite dialysis unit is situated at Newton Abbot (a community hospital); Cardiothoracic Surgery and Neuro-Surgery are provided by Regional Centre facilities at Plymouth, with consultants visiting Torbay. Torbay Hospital is a regional endoscopy training centre, bowel screening centre, and AAA screening centre, serving areas beyond its immediate South Devon catchment area. Torbay Hospital is approximately 2 miles from the centre of Torquay and all facilities are concentrated on the one site. The original hospital was built in 1928. The tower block area opened in 1970. More recently, major capital programmes have seen improvements in facilities for Cancer, Imaging, the Emergency Department and the opening of the Torbay Cardiac Centre. There has been significant capital investment in improvements in clinical areas and creation of significantly more single room accommodation. A young person’s unit is sited adjacent to the children’s inpatient facilities. The hospital is well supported by its League of Friends, which has given £1.5 million for the purchase of an intuitive surgical system (for robot-assisted procedures) and over £1.7 million to help build and equip our new Horizon Centre. The Horizon Centre opened in June 2009 as a state-of-the-art facility for healthcare innovation, education, training and research. The centre houses a significant clinical simulation facility. A varied programme of lectures, seminars and regular academic meetings are held. Torbay Hospital has an excellent reputation for undergraduate teaching and provides teaching for Peninsula Medical School students from years 3 – 5. The Trust has an established Education Directorate to support learning and has an energetic Research and Development Directorate with an extensive research portfolio. The current Director of Clinical Education is Dr Matt Halkes. Dr John Lowes is the Trust’s Medical Director. A 50-place Day Nursery for the children of staff has been purpose-built to expand our childcare provision. Holiday childcare schemes are also offered, together with extended opening hours. There is a Holiday Club available for children aged 4 – 11 years. The Trust has adopted the values set out in the NHS Constitution. All Trust employees are expected to own and exhibit these values in their work in the organisation. 3. COMMUNITY FACILITIES There are nine community hospitals in South Devon, in the Torbay Hospital catchment area, with whom we have excellent working relationships. All of the community hospitals are linked by PACS to the main radiology department at Torbay. Newton Abbot Hospital Torbay and Southern Devon Health and Care NHS Trust A new hospital opened at Newton Abbot in January 2009 providing 35 beds, a Reablement unit, Midwifery delivery facilities, Minor Injuries department, Out-patients department, Continence Advisory Centre which is an integral part of the well-regarded South Devon stroke service, Occupational Therapy and Physiotherapy and X-ray departments. There is a Satellite Dialysis Unit at the old hospital site. Radiology facilities in the new hospital comprise two digital radiography rooms and two ultrasound rooms (GE Logic E9). Teignmouth Hospital Torbay and Southern Devon Health and Care NHS Trust Teignmouth Hospital provides 17 GP beds and a newly upgraded Operating Theatre for local anaesthetic procedures. Other facilities include a Minor Injuries department, a recently expanded out-patients department and X-ray facility. Paignton Hospital Torbay and Southern Devon Health and Care NHS Trust Paignton Hospital has 28 beds providing GP Medical post operative, Rehabilitative and Palliative Care, and Consultant Care of the Elderly. Brixham Hospital Torbay and Southern Devon Health and Care NHS Trust Brixham Hospital is a 20-bed GP unit, with a clinic and community staff based on site, a Minor Injuries department and X-ray unit. Bovey Tracey Hospital Torbay and Southern Devon Health and Care NHS Trust 12 bed GP unit with a Minor Injuries unit and Physiotherapy unit. Dawlish Hospital Torbay and Southern Devon Health and Care NHS Trust The area’s newest community facilities, completed in 1999 under a PFI scheme, the 18-bed hospital is immediately adjacent to the town’s GP surgery. This has a Minor Injuries unit, reablement facilities, and a wide range of outpatient clinics and an X-ray unit. Ashburton and Buckfastleigh Hospitals Torbay and Southern Devon Health and Care NHS Trust A 16-bed GP unit with 2 district nurse beds. outpatient clinics. A Minor Injuries department, Physiotherapy unit and Dartmouth Hospital Torbay and Southern Devon Health and Care NHS Trust A GP unit with 16 beds with a Minor Injuries department, Physiotherapy an outpatients clinic and XRay unit. Totnes Community Hospital Torbay and Southern Devon Health and Care NHS Trust Totnes Community Hospital is a 19 bed GP unit, with a Minor Injuries Unit, X-ray facilities and speciality outpatient Clinics. Devon Partnership NHS Trust There are Community Mental Health and Learning Disability teams, Crisis and Outreach services, in-patient and Day Care units, Rehabilitation unit and a specialist Drug and Alcohol service within the South Devon area, managed by the Devon Partnership NHS Trust. 3. ADDITIONAL INFORMATION Clinical Governance and Risk Management The Trust believes everyone has a role to play in improving and contributing to the quality of care provided to our clients. THE OPHTHALMOLOGY DEPARTMENT We are a forward thinking, well equipped and well supported department. We provide a very wide range of ophthalmic services including intravitreal treatments for macular degeneration, vitreoretinal surgery, services for medical retina including uveitis, lacrimal surgery, glaucoma services, corneal services including PK, DSAEK and Intacs, paediatric services, adult motility and thyroid orbitopathy services, oculoplastics and of course cataract surgery. We have a talented and enthusiastic body of nurses, orthoptists and optometrists in the department trained to do glaucoma clinics, perform botox injections, treat lid cysts and perform electrolysis, give intravitreal Lucentis and provide sub-tenon’s blocks service for cataract surgery. The orthoptists and optometrists are involved in LogMAR vision testing, refraction and performing OCTs for the macular service including our research studies. We also have a full time ophthalmic photographer and an SAS doctor who is a contact lens specialist. We have just recruited a full time optometrist with contact lens experience, and have experienced sessional optometrists supporting the paediatric service. Our talented orthoptic department provide an enhanced service including paediatric LVA, a service for adult learning disabled patients, stroke rehabilitation and support for the macular clinics. We have recently embarked on a project to train orthoptists and optometrist to enable future involvement in the Virtual Medical Retina Clinics. There is an orthoptic led in-school vision screening scheme. Our surgical service is run from the purpose built Eye Surgery Unit (ESU) just upstairs from the Eye Clinic. From here we offer a one stop cataract assessment service, surgical booking service and admissions area. Our very few inpatients are admitted here and go to a ward postoperatively. The outpatient area includes 13 examination rooms (4 in ESU), a photography/FFA/ICG room with a full specification Spectralis, Cirrus and Stratus OCT, Humphrey and Goldman perimeters, Autorefraction, Orbscan, B scan and specular microscopy. Our laser suite within the clinic has an intravitreal injection room, plus rooms for our Pascal laser, PDT and brand new Yag laser with SLT capability. We hold clinics in community hospitals including Newton Abbot, Teignmouth, Dawlish, and Totnes. Newton Abbot Hospital also has a visual field and OCT facility. The Trust has a history of good working relationships with our commissioners and the Department has regular meetings with the commissioning bodies. There is a strong commitment to education both within Ophthalmology and within the Trust generally. There are strong links with the Peninsula College of Medicine and Dentistry (soon to split into two medical schools at Plymouth and Exeter) and we have students on placements both during the specialty block and for special study units (SSU). There is the opportunity to hold an honorary contract with the University by becoming involved in teaching or tutoring students. There is also the opportunity to become a mentor for a doctor in training. We have four specialty trainee doctors from the Peninsula Deanery in the department for a year at a time (2 at ST1-2 and 2 at ST 3-7). They rotate between Exeter, Plymouth, Truro, Taunton and Barnstable and are supported by our Department College Tutor as their Educational Supervisor. We have also run a TSC in Medical Retina. Training for Clinical and Educational Supervision is provided by the Peninsula Deanery (www.peninsuladeanery.nhs.uk). We are the only unit in the Peninsula to have an EyeSi VR surgical simulation trainer, and have been innovative in introducing it into training. We have published several papers in the field of surgical training using simulation. There is a full audit programme supported by the department of Clinical Effectiveness. We are a clinical placement school for undergraduate Optometrists and Orthoptists and have been involved in the newly set up programme for Assistant Practitioners. There is a strong research interest in the department and we are involved in a number of national trials, including IVAN, MERLOT, RELIGHT, INTREPID, LUMINOUS, studies in Pathological Myopia and Molecular Genetics in Glaucoma. We are also leading in a proposal for a UK multicentre trial on management of CSR. Retinal screening service There is a long standing highly regarded retinal screening programme in place. This is delivered in primary care premises using the facilities of the patient’s own GP practice and in community hospitals. There is an ongoing training programme for new screeners to diploma level. The medical retina specialists in the department provide clinical leadership to the service and a tertiary screening service. THE POST The post has arisen due to the relocation of one of our colleagues. We are looking for an enthusiastic, innovative general ophthalmologist with a special interest in glaucoma who is able to contribute to service design in a way that will enable us to deliver a high quality service as cost effectively as possible. There is a 1 in 7 on call commitment. There is a separate sub regional vitreoretinal on call. Shared office space and secretarial support will be provided. Outpatient activity currently occurs in Torbay and Newton Abbot Hospitals. In Torbay there are OCT, autofluorescence, FFA and ICGA facilities including the Spectralis as well as Cirrus and Stratus machines. We are in the process of introducing virtual clinics for a specific group of patients supported by doctors, orthoptists and optometrists. Lasering facilities are only available in Torbay where we have a new PASCAL and YAG lasers and facilities for SLT. Provisional Job Plan (Glaucoma) The job is offered as a 10 PA contract. The job plan is subject to annual review. We work flexibly to meet the needs of the department and definitive timetables are produced on a rolling 6 week cycle. The successful candidate will be expected to vary the proportion of general and subspecialist work according to the service needs of the department. The appointee will be required to work in one or more community clinics, depending on the needs of the service. Annual leave is 6 weeks plus 2 statutory days (i.e. 32 days) per annum plus bank holidays or time off in lieu. An increased basic annual leave entitlement is available for those consultants who have completed 7 or more years’ service as a consultant. At least 6 weeks notice is required for leave and must be planned within the constraints of cover arrangements. We have a departmental leave policy. By mutual agreement colleagues cover emergency on-call commitments and supervision of non consultants looking after absent colleagues’ routine work.
