IMPORTANT INFORMATION:
Dear registered patient,
Re: Relocation of Parkside Medical Centre branch surgery to Larkshall from 1st February 2017
We are writing to all our registered patients who attend at either the Larkshall Medical Centre or
Parkside Medical Centre to advise you that the branch surgery at Parkside Medical Centre will be
closing.
From 1st February 2017, you will no longer attend the Parkside Medical Centre for your medical care.

We have discussed the proposal to close the branch surgery with patients and the Waltham Forest
Clinical Commissioning Group. We will be able to operate more efficiently and you will have convenient
access to more services such as phlebotomy.
Your registration will remain with the Larkshall Medical Centre unless you decide you wish to register
elsewhere, which is your right and your choice. If you decide you would like to register with another
practice you will need to contact a local practice of your choice. At the back of this letter is useful
