New Office of ISCCM
We had initiated the purchase of new offices of Rs. 3.10 crores in the same building & on the same floor where we have our
existing office at Dadar during the tenure of Dr. Narendra Rungta. We are pleased to inform that the purchase process of this
offices has been completed and the new offices have been registered in the name of ISCCM during the year.
Indian Journal of Critical Care Medicine (IJCCM)
Achievements in the year 2014
1.
In the year 2014 IJCCM became monthly - All 12 issues were published on time
2.
Online issue is uploaded on the 5th of every month
3.
Print issue is mailed in the last week of every month. As we had fulfilled requirements, a highly subsidized mailing rate
(50 p. vs Rs. 20) from the postal department has been possible in the last 6 months.
4.
As compared to 256 submissions in the year 2012 and 350 in the year 2013, there have been 525 submissions in 2014.
5.
IJCCM is developing to be an international journal. There were 113 manuscripts submitted from outside India. Notably
there were 15 from the US, 10 from Europe and 22 from Iran among others which included China, Malaysia, Thailand,
Singapore, Australia, Sri Lanka, Nepal and Pakistan.
6.
Have been accepted for indexing in IndMed. Formalities are being fulfilled.
Areas for improvement
1.
The rejection rate has been 35% overall. By publishing standards this is very low and therefore more stringent criteria
need to be applied for acceptance.
2.
Should have IJCCM indexed on Medline in addition to PubMed. We feel that the requirements are being met. Shall soon
apply.
3.
Need to apply for impact factor. There are strict criteria for this certification. A premature application that gets rejected
would invite a moratorium on application for 2 years. Therefore adequate preparations are necessary.
4.
The weightage is more on research articles, systematic reviews and meta-analyses and narrative reviews. Weightage is low
for brief communications. Case reports only serve to dilute the impact factor (Since impact factor is calculated on the basis
of number of citations divided by number of publications).
5.
There is room to invite and to publish more research articles and reviews.
6.
In order to widen IJCCM readership image building and advertisements should be taken up more aggressively. I have
proposed to IJCCM that we should aim to hire a stall in major critical care / internal medicine / Anaesthesiology /
pulmonology congresses.
Dr. R K Mani
Chief Editor - IJCCM
Critical Care Communications (CCC)
CCC continued with all the regular features such as names of new members and branches approved during Executive
Committee Meetings, report of branches activities with their photos, addresses from the President and General Secretary etc.
The image challenge section was introduced. The small advertisements for job placements received were also published in
newsletter.

Editor-in-Chief, Critical Care Communications
Website
This year we have revamped the website and given completely new look. We have made an attempt to make it easy for user to
browse and go in any section of website without opening many pages. On the home page we have scrolling banners showcasing
important events and information regarding national conference.
We have redeveloped and streamlined the processes in new membership and institution software. On the front page we have
scrolling display of upcoming events and news and events updates. This makes it easy for a member to look at the events
happening all over the country. In the college and education tab, the important dates and information regarding various courses
offered by the Indian College of Critical Care Medicine is displayed.
Our publications, Indian Journal of Critical Care Medicine (IJCCM) and Critical Care Communications (Newsletter) are
available online. In the new website each branch has been given a space to upload their events by using a password. Our face
book, linked in and twitter pages are in the process of developing. We are also in the process of having payment gateway so that
it becomes easy for a member or student to pay online their membership or examination fees.

ISCCM Research Activities
Tropical Fevers in ICU
The study was conceived and planned by Dr N Rungta, Dr Ashish Bhalla and Dr Dhruva Chaudhry helped in drafting study
protocols and proforma.
Background: Infecions in Tropical and subtropical regions are often present with fever and organ failures and have overlapping
features. Specific etiology is almost indiscernible at admission whereas empiric therapy is warranted at outset. Paucity of
literature on this area made us to conduct this study to identify the clinical profile, etiology, treatment patterns and outcome of
tropical fevers admitted to ICUs in India.
Methods: Study design, setting and participants: This was a nation-wide multicentre prospective observational study
conducted in adult and pediatric ICUs between June 2014 and October 2015. All ICUs in the country were encouraged to
participate and enroll patients presenting with Fever without focus of >48 hours duration and onset <14 days with any of the
following: 1)Thrombocytopenia 2) Respiratory distress/ARDS 3) Jaundice 4) Encephalopathy 5) Renal failure 6) Multiorgan
failure. Data on demographic, clinical, laboratory and outcome were collected prospectively till discharge, death or 90 days of
hospital stay and were entered ‘on-line’ using a pre-tested format.
Results: A total of 456 patients were enrolled from 34 Intensive care units from different parts of India. Children younger than
12 years constituted 38% (n=173). Thrombocytopenia was the commonest presentation seen in 60% followed by respiratory
distress (46%), encephalopathy (29%), renal failure (24%), jaundice (20%). Nearly a fifth (19%) had multiorgan failure at
admission. An etiological diagnosis could be made in 367 (81.5%) cases. Dengue was the commonest tropical fever seen in
105 (23%) cases. Scrub typhus (n=83, 18%), meningitis and encephalitis (n=44, 9.6%), malaria (n=37, 8%) and bacterial
sepsis (n=32, 7%) were the other major diagnoses. Thrombocytopenia was seen with dengue (91%), malaria (65%) and scrub
typhus (60%). Patients with dengue had higher mean hemoglobin (12.8g/dl). Severe hypoalbuminemia (2.4g/dl) was a feature
of scrub typhus and sepsis. More than a third of patients with tropical fevers (n=160, 35%) received invasive mechanical
ventilation. A quarter (24%) was treated with vaso-active drugs and 9% required renal replacement therapy. At 28 days, 76.3%
survived without any disability and another 4.4% had some disability. 19.3% (80) patients died.
Conclusions: Dengue, scrub typhus, malaria, typhoid and bacterial sepsis common causes of tropical fevers presenting with
thrombocytopenia and/ or organ involvement. Point of care testing for dengue, malaria and typhoid can rule in or rule out these
diagnoses at admission and help in instituting specific therapy. Ceftriaxone with doxycycline or azithromycin would be
appropriate for empirical therapy while acyclovir may be considered in addition for patients presenting with encephalopathy
while waiting for laboratory reports.

Customised Health in Intensive Care, Trainable Research and Analysis Tool (CHITRA)
In May 2014 a proposal was put to the EC that ISCCM needs a customised system to collect and analyse data from various
centres to facilitate collaborative research. The system ideally should be cloud based and accessible by all ISCCM members. The
name CHITRA was suggested (Customised Health in Intensive care, Trainable Research and Analysis tool). The costs of
database development was discussed and a tentative plan for over 5 years was discussed.
The President of ISCCM Dr Shiva Iyer conducted a survey about scope and feasibility, following which Dr Shiva Iyer and Dr N
Ramakrishnan attended a presentation in Bangalore in which St John's Research Institute showcased their abilities to expedite
identification of a vendor and rollout of the system. Dr Kedar Radhakrishna of St John's Research Institute attended the next EC
as a special invitee and after his presentation the EC decided in principle to go ahead. A cost budget was prepared and handed
over to the president of the ISCCM. In principle the budget has been approved and the meeting with the vendors will be the next
step. To maintain privacy and confidentiality of patients, legal opinion was sought. Dr Shiva Iyer has requested Dr Dhruva the
General Secretary to get a legal opinion on safeguarding ISCCM interests.
Once the legal opinion has been obtained, vendor identification and finalisation of contract and construction of database will be
done. The vendor will roll out the database for trials within 180 days. It is planned that the data base can be showcased in the
Agra conference.
