Pet Store Management Certificate; or
I confirm that I (or a member of my staff namely ………………………………..) am in the course of
training to take the City and Guild Pet Store Management Certificate and shall have the qualification
within 2 years of the commencement of business as a licensed Pet Shop.

I enclose the sum of £189.60 being the amount of the fee payable on the Licence applied for.
I declare that neither I, nor any member of my staff, have ever been convicted of any offence relating
to cruelty to or mistreatment of animals or have at any time been disqualified from keeping a Pet
Shop.
