members. To determine whether the home is conducive
for home delivery. For the child whether it’s safe for the
child.
NURSES knows the family members of the mother or the
child. How many children in the family, who is well and
that need special attention? Who can be the support to
the mother and family in time of emergency?
Home Delivery, postnatal visits
These information will assist and guide for the nurse to
provide appropriate advise or health education
messages, anticipate the unexpected and assist mothers
and family to prepare for the unexpected events.

• Personalised care is an unwritten policy /
practice.
• It is practiced in Midwife Clinic (RBK) or
Klinik Desa (KD) by ‘default’, due to the
fact that there was only one or 2 nursing
staff in the KD.
• This was not practiced fully in Health
Clinics, because of convenience.

Increase the quality of client care where
the care is given by the same nurse .
Therefore there is consistency.
There is no continuity of care because
different nurses may differ in knowledge,
experiences , treatment plans and instruction
which may cause their clients to be confused,
and thus affect the trust of the clients to our
services.

PERSONALIZED CARE
Save time on client care, as nurses know their clients
especially the high risk cases.
Strengthen the rapport and trust with clients and
thus enhance the compliance and reduce the
incidence of defaulter
Less closeness with client due to frequent
change in nurses and this can cause
dissatisfaction to the clients especially when
questioned repeatedly on the same matter by
different nurses.
No long term plans for the clients
because nurses see clients on rotation
basis and there is a tendency for short
term plan only.

Nurses are more focused on cases under
their own supervision, thus ensuring
continuity of care.
Nurses are more thoughtful and serious in
client care, because they care from the
beginning of pregnancy till end of pregnancy
and so forth – ‘encouraging professional and
accountability.

Providing clients with information such
as test results or prompting them to
think about questions to ask before the
care planning consultation allows clients
to reflect and prepare to get the most
out of the discussion. It also establishes
an equal relationship with their nurses.

Delivering personalised care, listening to

Nurses may feel they are familiar with the
concept of care planning, but true
personalisation takes this to a new,
more dynamic level.
The care plan itself can also be a vehicle
for patients to hold important
information, a significant step in
shifting the locus of control.
