History of Pyxis®
•  An idea was born in 1987…
–  John McLaughlin, a LA doctor, came up with an idea of an automated
dispensing cabinet that could help reduce health care costs in the hospital
–  Ron Taylor became CEO in August of 1987
•  In 1990, Pyxis launched its Pyxis MedStation
®
system
•  In 1992, Pyxis decided to go public
•  In 1996, Cardinal Health acquired Pyxis® Corporation
•  In 2009, there was a spinoff resulting in CareFusion

•  Blind count – nurse is asked
to count and enter the
amount in the pocket
o  Recount prompt if first
recount entered does not
match expected count

main menu screen
•  Discrepancies can then be
researched

◦  interviewing the prior user
and/or reviewing the chart

Discrepancy Reports
•  ALL discrepancies should be resolved by the end
of the shift
‒  To document that there are no unresolved discrepancies,
the charge nurse can run an Open Discrepancy Report
at the end of the shift, review and sign the report and
leave it for the nurse manager to review.
•  Nurse managers review the All Discrepancies
Report daily to provide a “closed loop” system
‒  Do the reasons for resolved discrepancies make sense?
‒  Are the same users consistently creating discrepancies?
‒  Is there an educational or lack of compliance issue?

•  Weekly, the nurse manager chooses a controlled
substance to study
•  Run an Activities Report for the medication to be studied

Case Study #1:
A nurse is stealing medications upon removal from
the Pyxis MedStation® What reports can be run?
–  Discrepancy Report – will display date, time, user,
medication, pocket, and incorrect count
Service Messages – will identify the transactions
that were started at the Pyxis MedStation® but
subsequently cancelled
All Station Events Report – will give a complete
detailed listing of all activity performed at the Pyxis
MedStation®

How to catch diversion…
Case Study #2:
A nurse uses the override capability at the Pyxis
MedStation® in order to divert medications without
causing discrepancies or cancelled transactions.
What reports can be run?
–  Profile Override Report – can be run to find out what
medications a nurse has overridden for any patient. An
order must exist for that medication for that patient.
